祛痰活血汤联合氨氯地平治疗高血压(痰瘀互结)随机平行对照研究

Qutan Huoxue Decoction(祛痰活血汤) combined with Levamlodipine Benzenesulfonate in the Treatment of Hypertension(Tanyu Hujie/痰瘀互结)

[目的]观察祛痰活血汤联合氨氯地平治疗高血压(痰瘀互结)疗效。[方法]使用随机平行对照方法,将80例门诊按就诊顺序号方法随机分为两组。对照组40例氨氯地平,初始剂量为2.5mg/次,1次/d,根据临床反应,灵活增加剂量,最大可增至5mg/次,1次/d,早晨口服。治疗组40例祛痰活血汤(天麻、半夏各12g,茯苓、白术、薏苡仁、桃仁、红花、川芎各10g,白芷、当归、橘皮、菖蒲、姜生、大枣、甘草各6g),水煎400mL,早晚分服;氨氯地治疗同对照组。连续治疗1个月为1疗程。观测临床表现、收缩压、舒张压、不良反应。治疗1疗程(1个月),判定疗效。[结果]治疗组显效18例,有效19例,无效3例,总有效率92.50%;对照组显效9例,有效17例,无效14例,总有效率65.00%;治疗组疗效优于对照组(P<0.01)。收缩压、舒张压两组均有改善(P<0.01),治疗组改善优于对照组(P<0.05,P<0.01)。不良反应两组无显著差异(P>0.05)。[结论]祛痰活血汤联合氨氯地平治疗高血压(痰瘀互结),疗效满意,无严重不良反应,值得推广。

[Objective] To observe the curative effect of Qutan Huoxue decoction combined with L-amlodipine besylate on hypertension（Tanyu Hujie）. [Methods] Using random parallel control method, 80 outpatients were randomly divided into two groups according to the order number method. The control group of 40 cases of L-amlodipine besylate, the initial dose of 2.5 mg/time, once a day, according to clinical response, flexible increase the dose, the maximum can be increased to 5 mg/time, once a day, oral in the morning. In the treatment group, 40 cases were treated with Qutan Huoxue decoction（Tianma, Banxia each 12 g,Fuling,Baizhu, Yiyiren, Taoren, Honghua, Chuanxiong each 10 g, Baizhi, Danggui, Jupi, Changpu, Shengjiang, Dazao,Gancao each 6 g）, decocted in water for 400 mL in the morning and evening, and treated with levoamlodipine besylate as continuous treatment for 1 months for 1 courses. Clinical symptoms, systolic blood pressure,diastolic blood pressure and adverse reactions were observed. 1 courses of treatment（1 months） were used to determine the curative effect. [Results] In the treatment group, 18 cases were markedly effective, 19 cases were effective, 3 cases were ineffective, the total effective rate was 92.50%; in the control group, 9 cases were markedly effective, 17 cases were effective, 14 cases were ineffective, the total effective rate was 65.00%;the curative effect of the treatment group was better than that of the control group（P〈0.01）. Systolic blood pressure and diastolic blood pressure were improved in both groups（P〈0.01）. The improvement in treatment group was better than that in control group（P〈0.05,P〈0.01）. There was no significant difference in adverse reactions between the two groups（P〉0.05）. [Conclusion] Qutan Huoxue Decoction combined with L-amlodipine besylate in the treatment of hypertension（phlegm and blood stasis mutual knot） has satisfactory curative effect, no serious adverse reactions, and is worthy of promotion.
