Epidemiological characteristics and genotypes of human adenovirus among children in Xuzhou from 2013 to 2017

摘要 目的了解2013-2017年徐州市儿童腺病毒感染的流行情况及其分型特征。方法采集2013-2017年在徐州市儿童医院就诊的5177份流感样患儿的鼻或咽拭子。采用实时荧光PER方法进行腺病毒的检测,对阳性标本扩增其六邻体基因并测序进行型别分析。结果5177例患儿中共检出174例腺病毒阳性,检出率为3.36%。其中,3-6岁年龄组的患儿检出率最高,为5.30%(111/2095);全年各月份均有腺病毒检出,其中5月和6月为检出高峰,两月合计检出的腺病毒例数占检出总数的43.10%(75/174)。型别分析结果显示,本次研究共检出B、C和E共3组10种型别腺病毒,其中B组腺病毒所占比例最高,为80.46%(140/174),包括腺病毒3型107例,7型20例,55型8例和14型5例;其次为C组,占12.07%(21/174),包括1型13例,2型5例,腺病毒5型、6型和57型各l例;以及E组腺病毒4型,占7.47%(13/174)。结论2013—2017年徐州地区儿童呼吸道疾病腺病毒检出率为3.36%,感染以腺病毒3型和7型为主,其次为腺病毒1型和4型等。2013-2014年腺病毒流行以3型和7型为主,2015年后腺病毒3型逐渐成为优势型别。 Objective To study the epidemiological characteristics and genotypes of human adenovirus (HAdv) in Xuzhou from 2013 to 2017. Methods A total of 5 177 swap samples were collected from children with influenza-like illness in Xuzhou Children's Hospital during 2013-2017. HAdv was detected by Real-time PCR. Partial sequences of HAdV hexon gene were amplified from the positive samples, sequenced and genotyped. Results Among the 5 177 samples, 174 samples were tested positive of HAdV, and the detection rate was 3.36%. Among them, the highest detection rate was 5.30% (111/2 095) in the 3-6 years old group. Adenovirus was detected in each month, with peaks of detection in May and June, accounting for 43.10% (75/174) of all positive cases. The genotype analysis showed that 10 genotypes of adenoviruses belonged to B, C and E subgroups were detected. The proportion of subgroup B adenovirus was the highest, accounting for 80.46% (140 cases), including 107 cases of HAdV-3, 20 cases of HAdV-7, 8 cases of HAdV-55 and 5 cases of HAdV-14. The group C accounted for 12.07% (21 cases), including 13 cases of HAdV-1, 5 cases of HAdV-2, and 1 case each of HAdV-5, 6 and HAdVo57. HAdV-E4 accounted for 7.47% (13 cases). Conclusions The detection rate of adenovirusamong children with respiratory diseases in Xuzhou was 3.36% from 2013 to 2017. The predominant genotypes of HAdV infection were adenovirus genotypes 3 and 7, followed by genotypes 1 and 4. The prevalence of adenovirus in 2013-2014 were mainly genotype 3 and 7. After 2015, adenovirus genotype 3 gradually became the dominant type.
