摘要
内源性眼内炎是细菌或真菌通过远距离播散造成玻璃体炎为特征的眼内感染,视力预后差且具有一定死亡率。多重致病因素导致免疫水平低下者与正常的人群均有患病可能。临床表现缺乏特征性改变,传统微生物培养与分子诊断技术结合的快速诊断方式依然处于探索阶段。内源性眼内炎的治疗关键是明确原发病灶的同期进行系统与眼科专科治疗的衔接。需要以整合医学角度看待内源性眼内炎诊治,多科室联合挽救患者视力。
Endogenous endophthalmitis (EE) is intraocular infection results from hematogenous seeding of the eye by bacteria or fungi. Visual outcome was poor in the majority of cases and mortality rate were frequently observed. Because multiple pathogenetic factors play a role, immunocompromised and immunocompetent patients both have the risk of EE. Lack of characteristic features in clinical manifestations that require rapid diagnosis and treatment to save vision. Cultures of the vitreous, aqueous, blood cultures and molecular diagnostic techniques, those have been used in research laboratories for rapid diagnosis are still being explored. Treatment essential for endogenous endophthalmitis are systemic therapy for the underlying infection and intravitreal antibiotics simultaneously, some eases also benefit from a surgical vitrectomy. Integration of medicine provides the reasonable management for EE patients.
