中青年肠造口人性生活现状及影响因素分析

Study on the status and influencing factors of the sexual life in young and middle-aged colostomy patients

目的调查分析中青年肠造口人性生活现状及其影响因素,为肠造口人性生活质量干预提供参考依据。方法选取市内多所三甲医院121例中青年肠造口人,采用自编的一般资料调查问卷,亚利桑那州性体验量表(ASEX)、社会影响量表(SIS)及中文版造口患者自我效能量表(SSES)进行问卷调查。结果中青年肠造口人性体验得分为(21. 34±6. 074)分,性功能异常者有70例(57. 9%);患者性功能异常症状依次体现在性驱动(4. 76±1. 155)、性觉醒(4. 45±1. 291)、性高潮能力(4. 15±1. 333)、阴道润滑/阴茎勃起(4. 09±1. 402)、性高潮满意(3. 89±1. 454);患者性体验的影响因素包括家庭关系、性生活指导、家庭所在地、年龄、病耻感及自我效能(P <0. 05)。结论中青年肠造口人性问题突出,且受家庭关系、性生活指导、家庭所在地、年龄、病耻感及自我效能的影响。医护人员需重点关注中青年肠造口人的性生活问题,加强性教育,强调家属参与和同辈教育,帮助患者建立正确的价值观和性爱观,改善性生活应对状态,促进幸福感的获得,改善生活质量。

Objective To investigate and analysis the status and influencing factors of the sexual life in young and middle-aged colostomy patients,and to provide references to improve their qualities of sexual life. Methods A total of 121 young and middle-aged colostomy patients were investigated using the general information questionnaire,ASEX,SIS and SSES. Results The mean score of sexual experience was 21. 34 ± 6. 074 and 70 patients（ 57. 9%） had abnormal sexual experience. The symptoms of sexual experiences included sexual drive（ 4. 76 ± 1. 155）,sexual arousal（ 4. 45 ± 1. 291）,orgasm ability（ 4. 15 ± 1. 333）,vaginal lubrication/penile erection（ 4. 09 ± 1. 402）,and sexual satisfaction（ 3. 89 ± 1. 454）. The influencing factors of the sexual life in young and middle-aged colostomy patients included family relationship,sex life instruction,home place,age,stigma and self-efficacy（ P〈0. 05）. Conclusion The problem of the sexual life in young and middle-aged colostomy patients is prominent,which is affected by family relationship,sex life instruction,home place,age,stigma and self-efficacy. The medical staff should pay more attention to the sexual life in young and middle-aged colostomy patients,carry out sexual health education,family participation and peer education,and help them build correct values and sex views at the same time,as to promote happiness and improve the coping style of sexual life and the quality of life.
