枸橼酸氯米芬胶囊联合人绒毛膜促性腺激素治疗女性内分泌不孕症的效果

Effect of clomiphene citrate capsule combined with human chorionic gonadotropin on female endocrine infertility

目的:探究枸橼酸氯米芬胶囊联合人绒毛膜促性腺激素(human chorionic gonadotropin, HCG)治疗女性内分泌不孕的效果及对性激素水平的影响。方法:选取2016年8月至2017年8月于西宁市第一人民医院接受治疗的女性内分泌不孕患者,随机分为单药组和联合组。单药组患者使用枸橼酸氯米芬胶囊进行治疗,联合组患者在单药组治疗方法基础上使用HCG联合治疗。对2组患者治疗后排卵率、妊娠率、治疗效率和不良反应发生率进行对比。结果:本研究共纳入研究对象100例,单药组和治疗组各50例。治疗后联合组患者血FSH、LH、E_2、P、HO-1、NOS水平显著高于单药组,P<0.05;治疗后联合组卵巢体积、子宫内膜厚度均显著低于单药组,P <0.05;治疗后联合组患者的排卵率(96.00%)、妊娠率(82.00%)、治疗总有效率(96.00%)均显著高于单药组排卵率(82.00%)、妊娠率(66.00%)、治疗总有效率(82.00%),P <0.05;联合组不良反应发生率(2.00%)明显低于单药组不良反应发生率(14.00%),P <0.05。结论:使用枸橼酸氯米芬胶囊联合HCG对内分泌不孕患者进行治疗,能够明显改善性激素水平,提高排卵率和妊娠率,治疗效果显著。

Objective： To explore the effect of clomiphene citrate capsule combined with human chorionic gonadotropin （HCG） on the female endocrine infertility and the levels of sex hormone. Methods： The cases of endocrine infertile women in the hospital from August 2016 to August 2017 were selected, and randomly divided into the single drug group and the combined group. Cases in the single drug group were treated with clorniphene citrate capsule, while the combined group were treated with HCG combined with clomiphene citrate capsule. Enzyme-linked immunosorbent assay method was used to detect follicle-stimulating hormone （FSH）, luteinizing hormone generated （luteinizing hormone, LH） level expression; estradiol （E2） and progesterone （P） were detected by chemiluminescent immunoassay. The levels of heine oxygenase-1 （HO-1） and nitric oxide synthase （NOS） were detected by western blot The ovarian volume and endometrial thickness of the two groups were compared before and after treatinent. The ovulation rate, pregnancy rate, and the therapeutic efficiency and adverse reaction rate of the two groups was compared after treatment Results： A total of 100 cases were enrolled in with 50 cases ineach group. After treatment, the levels of FSH, LH, E2, P, HO-1, NOS in the combined group were significantly higher than that in the single group, P 〈0. 05. After treatment, the ovarian volume and endornetrial thickness of the combined group was signif- icantly lower than those of the single group, P 〈0.05. The ovulation rate （96.00% ）, pregnancy rate （82. 00% ）, and the total effective rate （96.00% ） in the combined group was significantly higher than those in the single group （82. 00%, 66.00%, 82.00% ）,P 〈0. 05. The incidence of adverse reactions （2. 00% ） in the combined group was significantly lower than that in the single group （14.00% ）,P 〈0.05. Conclusion. The method of clomiphene citrate capsule combined with HCG to treat female endocrine infertile patients can significantly improve the sex hormone levels, increase the rate of ovulation and pregnancy, and the therapeutic effect is obvious.
