1例抗癫痫药引起Stevens-Johnson综合征/中毒性表皮坏死症的病例分析

A case report of Stevens-Johnson syndrome/toxic epidermal necrosis caused by antiepileptic drugs

目的:探讨诱发Stevens-Johnson综合征(SJS)/中毒性表皮坏死松解症(TEN)的可能原因及防治方法。方法:分析本例SJS/TEN患者的临床表现、实验室检查、血药浓度监测及药物相关基因检测结果,同时通过文献检索,对该病例进行分析总结。结果:CYP2C19基因型为*1/*2,丙戊酸、苯巴比妥可能为SJS并最终发展为TEN的诱因。结论:TEN的诱因较多,死亡率高,感染、免疫性疾病与患者死亡率有较大的相关性。

Objective： To investigate the possible causes, prevention and treatment of Stevens-Johnson syndrome （SJS） /toxic epidermal necrolysis （TEN） . Methods： The clinical manifestations, laboratory tests, blood drug concentration monitoring and drug-related gene test results of the case of SJS/TEN were analyzed. Meanwhile, the literature was searched, and the case was analyzed and summarized. Results： CYP2C19 genotype was ＊ 1/＊ 2. Valproic acid and phenobarbital may be the inducement of SJS and TEN. Conclusion： SJS/TEN has many causes and high mortality. Infection and immune diseases are closely related to the mortality of patients.
