摘要
比较生物型全髋关节置换材料与骨水泥型人工髋关节材料围手术期TNF—α炎症指标变化,为临床选择髋关节材料类型提供参考依据。方法:回顾性分析我院2016年1月~2017年1月,63例高龄患者股骨颈骨折行人工全髋关节置换术,获得如年龄、性别、体温等,根据使用材料类型进行分组,分析TNF—α术前及术后之间的对比。结果:生物型全髋关节置换术(A组)33例,骨水泥型全髋关节置换术(B组)30例,A组与B组TNF—α术后比较明显较低,差异有统计学意义(P〈0.05)。A组与B组分别进行组内比较,TNF—α术后较术前明显升高,差异有统计学意义(P〈0.05)。结论:使用生物型全髋关节置换材料术后血清TNF—α升高幅度明显要低于使用骨水泥型人工髋关节材料,从手术创伤炎症角度考虑应尽量使用生物型全髋关节材料。
Objective: To compare the changes of TNF—α inflammaUon between biological total hip replacement matenats anu bone cement artificial hip joint materials during perioperative period. Methods: Retrospective analysis of 2016 January to 2017 January, artificial total hip replacement in 63 elderly patients with femoral neck fracture, according to the types of materials used, the comparison between pre operation and post - operation of TNF—α was analyzed. Results: Total hip replacement group A ( n = 33 ), total hip replacement group B ( n = 30). The postoperative TNF—α in group A and B was significantly lower than that in group B ( P 〈 0.05 ). The difference between group A and group B was statistically significant ( P 〈 0.05 ). The level of TNF—α after operation was significantly higher than that before operation. The difference was significant ( P 〈 0.05 ). Conclusion: The increase of serum TNF—α after using biological total hip replacement material was obviously lower than that of bone cement artificial hip joint material. Biological total hip joint materials should be used as far as possible from the perspective of surgical trauma inflammation.
