209例大动脉粥样硬化性脑卒中的危险因素分析

Risk Factor Analysis of 209 Cases of Atherosclerotic Stroke

目的通过分析209例大动脉粥样硬化性脑卒中的危险因素,为脑卒中的防治提供临床依据。方法选择2016年1月-2017年12月收治的209例大动脉粥样硬化性脑卒中患者完成病史采集,并进行NIHSS评分评定,行头颅核磁检查,每例患者出院时进行改良Bathel指数评定量(MBI)表进行评分,MBI<60分为生活功能障碍组,MBI≥60分为生活基本自理。对前后二组的危险因素进行多元回归分析。结果共选209例,前循环134例,后循环75例。前后循环二组中糖尿病、饮酒有统计学意义。多因素逻辑回归分析前循环组中MBI等级与年龄、糖尿病、吸烟、NIHSS的有统计学意义。在后循环组中MBI等级与危险因素和NIHSS评分无明显相关性。结论常见动脉粥样硬化性脑卒中危险因素对前后循环的预后的影响不同。

Objective The purpose of this study was to provide a clinical basis for the prevention and treatment of stroke,by analyzing the risk factors of 209 cases of atherosclerotic stroke. Methods Selection of 209 cases of atherosclerotic stroke patients admitted by our department from January 2016 to December 2017,and conducted the NIHSS score evaluation,and the nuclear magnetic resonance imaging. Each patient was discharged with a modified Bathel index（MBI）scale,and MBI 60 was divided into the life dysfunction group,and MBI was more than 60 divide into basic self-care. Multiple regression analysis was performed on the risk factors of the two groups. Results 134 cases were in the anterior circulation and 75 in the posterior circulation. Diabetes and alcohol consumption were statistically significant in both groups. The MBI level and age,diabetes,smoking and NIHSS were statistically significant in the anterior circulation group. There was no significant correlation between MBI level and risk factor and NIHSS score in the posterior circulation group. Conclusions The risk factors of common atherosclerotic stroke have different effects on the prognosis of anterior and posterior circulation.
