髋关节镜下骨赘切除联合盂唇修补治疗合并髋臼股骨撞击症的盂唇损伤的中长期随访结果

The Medium-to Long-Term Results of Labral Repair Combined with Arthroscopic Correction of Femoroacetabular Impingement for the Treatment of Labral Injuries with Femoroacetabular Impingement

目的:探讨髋关节镜下骨赘切除联合盂唇修补治疗合并髋臼股骨撞击症的盂唇损伤的中长期疗效。方法:回顾性分析2010年1月至2012年12月北京积水潭医院运动医学科采用髋关节镜下骨赘切除联合盂唇修补治疗合并髋臼股骨撞击症的盂唇损伤患者106例的临床结果。这106例患者中,共91例完成2年随访,平均随访时间26.0±2.0月(24～32月),64例完成至少5年随访,平均随访时间62.9±3.1月(60～70月)。分别于术前、术后2年、术后5年对患者进行改良Harris髋关节评分(m HHS)、VAS疼痛评分,于术后2年和术后5年进行患者满意度评分。结果:106例患者术前改良Harris髋关节评分为54.0±12.5分(34～76分),VAS评分为6.3±1.9分(3～9分);术后2年改良Harris髋关节评分为83.9±10.0分(59～100分),VAS评分为2.4±1.7分(0～6分),患者满意度为8.3±1.3分(5～10分);术后5年改良Harris髋关节评分为80.8±12.3分(53～100分),VAS评分为2.5±1.9分(0～7分),患者满意度为8.0±1.5(4～10分)。术后2年、术后5年与术前相比,改良Harris髋关节评分和VAS评分均有显著改善(P=0.000);术后5年与术后2年相比,改良Harris髋关节评分、VAS评分和患者满意度无显著性差异(P=0.140,0.747,0.346)。结论:中长期随访结果显示,关节镜下盂唇修补联合骨赘切除治疗合并股骨髋臼撞击症的盂唇损伤可以取得满意的临床效果。

Objective To explore the medium-to long-term results of labral repair in the patients who underwent arthroscopic correction of femoroacetabular impingement（FAI）. Methods Totally 106 patients undergoing labral repair combined with arthroscopic correction of FAI in our hospital between January 2010 and December 2012 were reviewed retrospectively. Among them,91 patients finished a follow-up of no less than 2 years,with an average of 26.0 ± 2.0 months（24-32 months） and 64 completed a follow-up more than 5 years,with an average of 62.9 ± 3.1 month（60-70 months）. Before as well as 2 and 5 years after the operation,all patients were measured using the modified Harris hip score（mHHS） and the visual analog scale（VAS） for pain,as well as their satisfaction. Results There were no significant differences in the average mHHS,VAS and patient satisfaction at 2-and 5-year postoperative follow-up[（83.9 ± 10.0）（59-100）,（2.4 ± 1.7）（0-6） and（8.3 ± 1.3）（5-10）;（80.8 ±12.3）（53-100）,（2.5 ± 1.9）（0-7） and（8.0 ± 1.5）（4-10）],but showing significant improvement compared with the preoperative mHHS[（54.0 ± 12.5）（34-76）] and VAS [（6.3 ± 1.9）（3-9）]. Conclusion Labral repair in the patients who underwent arthroscopic correction of FAI shows satisfied clinical results at the medium-and long-term follow-up.
