静态渐进性牵伸技术联合综合康复治疗骨折后膝关节僵硬的疗效观察 被引量:33

摘要 目的:探讨静态渐进性牵伸技术(SPS)联合综合康复治疗对骨折后膝关节僵硬患者功能恢复的影响。方法:选取因骨折造成膝关节僵硬的患者80例,随机分为观察组(40例)和对照组(40例)。两组均予物理因子、关节松动术、运动疗法等综合康复疗法,观察组在此基础上采用JAS支具进行静态渐进性牵伸治疗。分别于治疗前、治疗8周后对患者进行美国特种外科医院膝关节评分(HSS)、膝关节主动活动度(AROM)、临床疗效评定。结果:共统计了观察组(33例)和对照组(31例)共64例患者。两组患者的AROM、HSS评分在治疗8周后均较治疗前提高,差异有显著性意义(P<0.05);观察组HSS评分、AROM分别为(81.4±3.2)分和115.7°±15.7°,较对照组有更明显的改善(P<0.05);观察组的显效率为78%,优于对照组(P<0.05)。结论:应用静态渐进性牵伸技术联合综合康复能够有效改善骨折后膝关节僵硬,促进膝关节功能恢复。 Objective: To evaluate the effect of static progressive stretch combined with comprehensive rehabilitation for the treatment of knees stiffness after fracture. Method: Patients with knees stiffness caused by fracture were randomly allocated to the observation group(33 cases) and the control group(31 cases). Two group were treated with physical agents, joint mobilization, exercise therapy and proprioception training. Besides, the observation group were treated with static progressive stretch (JAS knees orthosis). HSS knee score, efficacy and active knees joint mobility (AROM) were evaluated for the two groups before and after the 8 week rehabilitation program. Result: HSS knee score and active knees joint mobility (AROM) of two groups improved more significantly after 8 weeks treatment as compared with those before treatment (P〈0.05). HSS knee score and AROM of observation group were significantly better than control group (P〈0.05). Conclusion: Static progressive stretch orthoses combined with comprehensive rehabilitation could promote the knee function recovery of knee stiffness patients more than comprehensive rehabilitation only.
