盐酸右美托咪定预处理对创伤骨科患者术后疼痛控制效果的影响

Effect of Postoperative Pain control in Trauma Osteopathic Patients who are Pretreated with Dexmedetomidine Hydrochioride in Painless Ward

目的：探讨盐酸右美托咪定预处理对创伤骨科患者术后疼痛控制效果的影响.方法：选择60例创伤骨科需行手术患者为研究对象,随机分为2组：盐酸右美托咪定组﹙D组n=30﹚和常规处理组﹙N组n=30﹚;所有患者入室后均开放外周静脉,D组给予盐酸右美托咪定1μg/kg经静脉泵注（15分钟）;N组给予常规诱导,术后均给予帕瑞昔布钠40mg静脉滴注,2次/d.观察并比较2组患者术后1小时﹙T0﹚、3小时﹙T1﹚、6小时﹙T2﹚、12小时﹙T3﹚、24小时﹙T4﹚、48小时﹙T5﹚及术后72小时﹙T6﹚2组患者疼痛评分、解救药使用情况及患者满意度.结果：2组患者疼痛评分在T0无明显差异﹙P﹥0.05﹚,D组T1～T6均较N组偏低﹙P﹤0.05﹚;D组T0～T6解救药使用次数较N组明显偏少﹙P﹤0.05﹚;且D组患者满意度较N组明显偏高﹙P﹤0.05﹚.结论：对创伤骨科手术患者,盐酸右美托咪定术前预处理不仅能明显降低患者术后疼痛的发生,减少解救药物的使用,还能显著提高患者的舒适满意度.

Objective： To explore the effect of postoperative pain control in trauma osteopathic patients who are pretreated with dexmedetomidine hydrochloride in painless ward. Methods： Sixty osteopathic patients undergoing surgery were randomly divided into two groups ： Dexmedetomidine hydrochloride group（ group D, n = 30） and normal treated group （ group N, n = 30） . All patients were venipunctured after entering operation rooms, D groups receiving continuous IV infusion of dexmedetomidine hydrochloride at 1.9 g/kg during 15minutes, N groups were receiving normal anesthesia. All patients were treated with Parecoxib Sodium 40mg through IV infusion 2 times per day. Pain score .Rescue drugs use and Satisfaction of patient were served. Observe and recorded the time postoperation lh （ T0 ） , 3h （T1） ,6h （T2） ,12h （T3） ,24h （T4） ,48h （T5） and72 （T6）. Results： Pain score had no significant differences at TO between the two groups （ P 〉 0.05 ） , but significantly decreased at T3 ～T6 in D group. Rescue drugs use in D groups obviously decrease compared with N groups （ P 〈 0.05 ） . Satisfaction of patient was significantly increased in D group compared with iN groups （ P 〈 0.05 ） . Conclusion： To Trauma osteopathic patients , the dexmedetomldine hydrochloride pretreated not only can decrease the pain score and reseue drugs use obviously , but also increase the satisfactions of patients significantly.
