血清淀粉样蛋白A和可溶性弹力蛋白与冠状动脉扩张的相关性 被引量:4

Correlation between serum amyloid A,soluble elastin and coronary artery ectasia

摘要 目的:探究血清淀粉样蛋白A和可溶性弹力蛋白在冠状动脉(冠脉)扩张中的作用。方法:回顾性分析我院心内科冠脉介入影像资料,随机选取30例冠脉扩张患者作为冠脉扩张组,同时随机选取30例冠心病患者(冠心病组)和30例冠脉正常患者(对照组)为对照。收集患者一般临床资料以及生化指标,检测血清淀粉样蛋白A、可溶性弹力蛋白水平。Logistic回归、ROC曲线分析冠脉扩张的发病相关危险因素和有效的预测因子。结果:3组在性别、糖尿病病史、高密度脂蛋白胆固醇(HDL-C)、超敏C反应蛋白、血肌酐、同源半胱氨酸方面有显著统计学差异(P<0.05)。冠脉扩张组血清淀粉样蛋白A、可溶性弹力蛋白水平与冠心病组、对照组比较,均差异有统计学意义(P<0.05)。Logistic回归分析发现,高水平血清淀粉样蛋白A和可溶性弹力蛋白为冠脉扩张发生发展的独立危险因素(血清淀粉样蛋白A:OR=1.001,95%CI:1.000~1.002,P=0.007;可溶性弹力蛋白:OR=1.199,95%CI:1.027~1.400,P=0.022)。血清淀粉样蛋白A、可溶性弹力蛋白的ROC曲线下面积分别为0.859、0.809。结论:血清淀粉样蛋白A、可溶性弹力蛋白是冠脉扩张形成的独立危险因素,且血清淀粉样蛋白A可以更有效地预测冠脉扩张的发生。 Objective:To explore the role of serum amyloid A and soluble elastin in the development of coronary artery ectasia.Method:We retrospectively analyzed the imaging data of patients with diagnostic coronary intervention in our hospital.The 30 patients with coronary artery ectasia were randomly selected as the ACE group,30 patients with coronary atherosclerosis disease as CAD group and 30 patients without coronary heart disease as the control group.General clinical data and biochemical indexes were collected to detect serum amyloid A and soluble elastin levels.The risk factors and effective predictors of coronary artery ectasia were analyzed by Logistic regression and ROC curve.Result:There were statistically significant differences in gender,diabetes history,HDL-C,hypersensitive C-reactive protein and serum creatinine among the three groups(P〈0.05).Serum amyloid A and sElastin levels increased in CAE group than control group or CAD group(P〈0.05).Logistic regression analysis showed that high level serum amyloid A was an independent risk factor for the development of coronary artery ectasia(serum amyloid A:OR=1.001 95%CI1.000-1.002 P=0.007;sElastin:OR=1.199 95%CI1.027-1.400 P=0.022).The area under the ROC curve of serum amyloid A and sElastin were 0.859 and 0.809 respectively.Conclusion:Serum amyloid A and soluble elastin are independent risk factors for coronary artery ectasia.Serum amyloid A can effectively predict the occurrence of coronary artery ectasia.
