优质护理模式对经内镜逆行胰胆管造影术患者护理质量的影响

Effect of Quality Nursing Mode on Quality of Care in Patients Undergoing Endoscopic Retrograde Cholangiopancreatography

目的探讨分析优质护理模式对经内镜逆行胰胆管造影术(ERCP)患者护理质量的影响。方法将2016年5月—2017年12月盐城市第一人民医院72例ERCP患者分为两组各36例,对照组实施常规护理,观察组实施优质护理模式,评估两组患者的护理效果。结果两组患者术前睡眠、术中血压异常变化、术中有无中止手术、患者配合度、术后进食情况和满意度差异有统计学意义(P<0.05);不同护理模式对术后胰腺炎发病率的影响并无明显差异(χ~2=1.014,P=0.500);对两组数据进行Logistic多元回归分析,结果显示,优质护理模式下对ERCP患者术前睡眠状况、术后睡眠状况、患者依从性、术中血压变化、手术中止、术后擅自禁食以及术后满意度等因子有正向的影响作用(β=-0.559,-1.776,-0.903,-2.023,-1.477,-0.942,-1.803,RR=0.619,0.429,0.411,0.229,0.307,0.181,1.176,P=0.012、0.029、0.026、0.021、0.022、0.027、0.020)。结论优质护理模式可有效改善ERCP患者围手术期睡眠质量,控制血压波动,提高患者依从性和手术成功率,对患者术后依从性和满意度有良好的改善效果,从整体上改善护理工作质量。

Objective To investigate the effect of quality nursing mode on the quality of care in patients undergoing en- doscopic retrograde cholangiopancreatography （ERCP）. Methods From May 2016 to December 2017, 72 patients with ERCP in Yancheng First People＇s Hospital were divided into two groups of 36 patients. The control group received rou- tine nursing care. The observation group implemented quality nursing mode to evaluate the nursing effect of the two groups. Results There were significant differences in preoperative sleep, abnotanal intraoperative blood pressure, intra- operative or non-surgery, patient satisfaction, postoperative feeding and satisfaction between the two groups （P〈0.05）. There was no significant difference in the incidence of morbidity of different nursing modes for postoperative pancreati- tis （χ2=1.014, P=0.500）. Logistic multiple regression analysis was performed on the two groups of data. The results showed that preoperative sleep status, postoperative sleep status, and patient compliance in ERCP patients under high quality nursing mode. Factors such as sexual, intraoperative blood pressure changes, surgical suspension, postoperative fasting, and postoperative satisfaction have positive effects （β=-0.559, -1.776, -0.903, -2.023, -1.777, -0.942,- 1.803, RR = 0.619, 0.429, 0.411, 0.229, 0.307, 0.181, 1.176, P=0.012, 0.029, 0.026, 0.021, 0.022, 0.027, 0.020）. Conclusion Quality nursing mode can effectively improve the perioperative sleep quality of ERCP patients, control blood pressure fluctuations, improve patient compliance and success rate, improve the patient＇s postoperative compliance and satisfac tion, and improve the overall nursing work quality.
