他汀类药物在动脉粥样硬化性缺血性脑卒中患者中的应用及依从性调查

Application and compliance of statins in patients with atherosclerotic ischemic stroke

目的了解他汀类药物在动脉粥样硬化性缺血性脑卒中患者中的使用情况,分析影响他汀类药物依从性的原因。方法选择2016-07—2017-07在郑州大学附属洛阳中心医院神经内科住院的动脉粥样硬化性缺血性脑卒中患者687例为研究对象,根据患者入院时是否服用他汀类药物分为服药组212例和未服药组475例,对影响患者他汀服药依从性的相关因素进行Logistic回归分析。结果本研究中他汀服药率为30.86%,影响他汀依从性的独立危险因素为:高脂血症、冠心病、糖尿病、抗血小板药物应用、吸烟史(P<0.05);未服他汀类药物的原因中,前5位为病情恢复血脂正常、自觉服药无作用、经济原因、担心不良反应、医生未建议。结论缺血性脑卒中患者他汀类药物服药率低,服药剂量不达标;应进一步普及指南要求,提高患者他汀类服药的依从性。

Objective Learn about the use of statins in patients with atherosclerotic ischemic stroke and analyze the causes of statin compliance. Methods 687 patients with atherosclerotic ischemic stroke admitted to our hospital from July 2016 to July 2017 were selected as subjects.According to whether patients took statins at admission,they were divided into Medication group （212 cases）and non medication group（475 cases）.Using the Logistic regression analysis to analyse the risk factor between the two groups. Results The statin rate was 30.86% in this study.The independent risk factors for statin compliance were：hyperlipidemia,coronary heart disease,diabetes,antiplatelet drug use,smoking history （ P 〈0.05）.The top five reasons of non adherence with statins were the recoverment of disease,doubted the necessity of using statins,economic pressure,worries about side effect and no recommendations from doctors. Conclusion In patients with ischemic stroke,the rate of statin medication is low,and the dosage is not up to standard;the guidelines should be further popularized to improve the compliance of patients with statin medication.
