右美托咪啶对腹腔镜胆囊切除老年患者应激反应的影响

Effect of Dexmedetomidine on Stress Response in Elderly Patients Undergoing Laparoscopic Cholecystectomy

目的研究探讨右美托咪啶对腹腔镜胆囊切除老年患者应激反应的影响。方法该研究方便选取2017年3月—2018年6月在该院进行以规定研究时段内收治的60例胆囊切除老年患者为研究对象,随机均分两组,各30例,以丙泊酚麻醉为对照组,以右美托咪定麻醉为观察组,分析右美托咪啶对腹腔镜胆囊切除老年患者应激反应的影响,并对比两组的不良反应发生情况。结果观察组不良反应发生率为6.7%,对照组为16.7%,两组比较差异有统计学意义(X^2=4.073,P=0.025),观察组插管后的HR和对照组比较(t=4.158,P=0.001 0);SpO2和对照组比较(t=4.425,P=0.0024);MAP和对照组比较(t=4.015,P=0.001 7);NE和对照组比较(t=4.338,P=0.003 8);应激反应指标比对照组的要稳定,差异有统计学意义(P<0.05)。结论使用右美托咪啶对腹腔镜胆囊切除老年患者进行麻醉,可以帮助患者减少不良反应的出现,从而减轻患者的应激反应,效果显著,值得推广应用。

Objective To investigate the effect of dexmedetomidine on stress response in elderly patients undergoing laparoscopic cholecystectomy. Methods The study was conducted in the hospital from March 2017 to June 2018. The 60 patients with cholecystectomy who were admitted during the study period were conveniently selected and randomly divided into two groups, 30 patients in each group, anesthetized with propofol. For the control group, dexmedetomidine anesthesia was used as the observation group to analyze the effect of dexmedetomidine on the stress response in elderly patients undergoing laparoscopic cholecystectomy, and the adverse reactions of the two groups were compared. Results The incidence of adverse reactions was 6.7% in the observation group and 16.7% in the control group. The two groups were compared the difference was statistically significant（X^2=4.073, P=0.025）. The HR in the observation group was compared with the control group（t=4.158, P=0.001 0）. SpO2 was compared with the control group（t=4.425, P=0.002 4）; MAP was compared with the control group（t=4.015, P=0.001 7）; NE was compared with the control group（t=4.338, P=0.003 8）; stress response index was more stable than the control group. Conclusion Anesthesia in elderly patients undergoing laparoscopic cholecystectomy with dexmedetomidine can help patients reduce the occurrence of adverse reactions, thereby reducing the stress response of patients, and the effect is significant, which is worthy of popularization and application.
