摘要 目的分析彩色多普勒超声诊断颈动脉体瘤的效用,总结诊断经验。方法 2011年1月—2017年1月,医院共诊断CBT患者18例,超声主要围绕颈部以及包块进行超声检查,进行肿块的分析,同时对压迫的颈动脉进行纵、横向检查,分析是否存在管壁内膜中层厚度上升、斑块形成等合并的颈动脉病变。结果颈总动脉分叉处88.9%(16/18),边界平整94.4%(17/18),压迫颈内以及颈外动脉出现明显的移位率(83.3%),表现为开口角度改变占17.8%。1例病灶较小,显示不清,超声诊断符合率94.4%(17/18),未见明显的异常。随着分级的上升,脑缺氧症状、饮水呛咳以及声音嘶哑症状出现率逐渐上升(X^2=8.08、6.69,P=0.018、0.035<0.05)。结论超声诊断颈动脉体瘤效果较好,诊断不明主要与病灶较小、显示不清有关;超声能够准确的分型,通过分析边界、部位、开口角度,分析颈动脉体瘤与相关症状之间的关系,为治疗提供依据。 Objective To analyze the utility of color Doppler ultrasound in the diagnosis of carotid body tumor and to summarize the diagnostic experience. Methods From January 2011 to January 2017, 18 patients with CBT were diagnosed in the hospital. Ultrasound was performed on the neck and mass, and the mass was analyzed. The carotid artery was compressed for longitudinal and lateral examination. Whether there was a combined carotid artery lesion such as an increase in the thickness of the intima of the intima of the wall and plaque formation. Results The common carotid artery bifurcation was 88.9%(16/18), and the boundary was flattened by 94.4%(17/18). The compression of the neck and the external carotid artery showed a significant shift rate 83.3% and 17.8%; One lesion was small and showed unclear. The coincidence rate of ultrasound diagnosis was 94.4%(17/18), and no obvious abnormality was observed. As the grading increased, the incidence of cerebral hypoxia, water cough, and hoarseness increased gradually(X^2=8.08, 6.69, P=0.018, 0.035〈0.05). Conclusion Ultrasound diagnosis of carotid body tumor is effective. The diagnosis is mainly related to small lesions and unclear findings.Ultrasound can accurately classify and analyze carotid body tumors and related symptoms by analyzing the boundary, location and opening angle. The relationship between the two provides a basis for treatment.
