摘要
目的探讨超声心动图在观察糖尿病前期和2型糖尿病患者左心形态及功能的变化中的价值。方法选取我院糖尿病前期患者30例(糖尿病前期组)、2型糖尿病患者32例(2型糖尿病组),以及健康成人31例(健康对照组),应用超声心动图测量各组左室舒张末内径、室间隔舒张末厚度(IVSd)、左室后壁舒张末厚度(LVPWd)、左房容积指数(LAVI)、左室射血分数(LVEF),以及二尖瓣环组织多普勒频谱e’峰、a’峰、s’峰平均值(e’mean、a’mean、s’mean),计算二尖瓣口舒张期血流频谱E峰与二尖瓣环组织多普勒频谱e’峰比值(E/e’mean),比较各组上述参数的差异。结果健康对照组与2型糖尿病组LAVI、IVSd、LVPWd、s’mean、e’mean和E/e’mean比较,差异均有统计学意义(均P<0.01);健康对照组与糖尿病前期组LAVI、IVSd、s’mean、e’mean和E/e’mean比较,差异均有统计学意义(均P<0.05);2型糖尿病组与糖尿病前期组LAVI、IVSd、s’mean、e’mean和E/e’mean比较,差异均有统计学意义(均P<0.05)。结论超声心动图可准确评价糖尿病前期和2型糖尿病患者左心形态和功能的变化情况,具有一定临床应用价值。
Objective To observe the changes of left ventricular morphology and function in patients with prediabetesand diabetes mellitus by echocardiography.Methods Thirty cases with prediabetes (prediabetic group), 32 cases with diabetesmellitus (type2 diabetes group)and 31 cases of healthy people (control group)were enrolled in this study.The echocardiographicparameters measured included: left atrial volumn index (LAVI), the end-diastolic thickness of interventricular septum (IVSd), leftventricular end-diastolic dimension (LVEDD), left ventricular posterior wall end-diastolic thickness (LVPWd), left ventricularejection fraction (LVEF), myocardial velocity during mitral annulussystolic, early and late diastole phase respectively (e’ mean, a’ meanands’ mean) .The blood flow velocity of mitral annulus during early diastole (E)and the peak velocity of mitral annulus by Dopplertissue imagimg (e’ )were also measured, and E/e’ ratio was calculated (E/e’ mean) .The indicators were analyzed statistically betweenthe groups.Results Compared with the control group, there were significant differences in LAVI, IVSd, LVPWd, s’ mean, e’ mean,e’ mean and E/e’ mean in type2 diabetes group (all P〈0.01) .Compared with the control group, there were significant differences in LAVI,IVS, s’ mean, e’ mean and E/e’ mean in the prediabetic group (all P〈0.05) .Compared with the prediabetic group, there were significantdifferences in LAVI, IVSd, s’ mean, e’ mean and E/e’ mean in type2 diabetes group (all P〈0.05) .Conclusion Echocardiography canaccurately evaluate the changes of left ventricular morphology and function in patients with prediabetes and diabetes mellitus, ithas certain clinical application value.
