摘要
目的探讨超声评价前列腺中叶增生患者症状的应用价值。方法对223例良性前列腺增生患者进行国际前列腺症状评分(IPSS),应用超声测量前列腺体积(PV)、中叶突入膀胱距离(IPP)及残余尿量(RUV),根据不同IPP值分为3组:A组43例(IPP≥20 mm),B组91例(IPP为11~19 mm),C组89例(IPP≤10 mm),比较各组间IPSS、RUV的差异,并分析IPP与IPSS、RUV的相关性。结果各组IPSS、RUV比较差异均有统计学意义(均P<0.01);组间IPSS两两比较差异均有统计学意义(均P<0.01);A组与C组、B组与C组RUV比较,差异均有统计学意义(均P<0.01)。A组IPP与IPSS、RUV均呈中低度正相关(r=0.547、0.420,均P<0.01);A、B、C组IPSS与RUV均呈高度正相关(r=0.905、0.869、0.963,均P<0.01)。结论应用超声测得的IPP对于评价前列腺中叶增生患者尿路梗阻症状有较大临床意义。
Objective To explore the application value of ultrasound in the evaluation of symptoms of prostatic middlelobe hyperplasia patients.Methods A total of 223 cases of benign prostatic hyperplasia(BPH)patients were evaluated byinternational prostate symptom score(IPSS), the prostate volume(PV), intravesical prostatic protrusion(IPP)and residual urinevolume (RUV)were measured by ultrasonography.According to different IPP, the patients were divided into3 groups: groupA (n=43, IPP逸20 mm), groupB (n=91, IPP was 11-19 mm)and groupC(n= 89 cases, IPP臆10 mm) . IPSS and RUV werecompared to find out whether there were significant differences between different groups.The correlations between IPP and IPSS,IPP and RUV were analyzed as well.Results The difference of IPSS and RUV were statistically significant in each group (allP〈0.01) .The IPSS was significant differences between any two groups (all P〈0.01) .Comparison of RUV between group A andC,group B and C showed statistically significant differences (all P〈0.01) .In groupA, IPP was moderately correlated with IPSSand RUV (r=0.547, 0.420, all P〈0.01) .IPSS of groupA, B and C were highly correlated with RUV (r=0.905, 0.869, 0.963,all P 〈0.01) .Conclusion IPP measured by ultrasound is of great clinical significance for the evaluation of urinary tractobstruction in prostatic middle lobe hyperplasia patients.
