经颅多普勒超声在前列地尔治疗后循环缺血性眩晕疗效评估中的应用

Application of TCD ultrasound on therapeutic effect evaluation of alprostadil for patients with PCIV

目的:探究前列地尔对后循环缺血性眩晕(PCIV)的疗效,以及经颅多普勒(TCD)超声的使用价值。方法:选取90例PCIV患者,将其采用数表法随机分为观察组与对照组,每组45例。对照组给予基础治疗,观察组在基础治疗上加用前列地尔注射液治疗。对患者治疗前后眩晕症状的好转程度进行评分;测量椎动脉(VA)和基底动脉(BA)的平均峰流速(Vm),对比两组患者治疗前后差异。结果:治疗后观察组眩晕症状评分为(0.27±0.54)分,显著低于对照组的(0.69±0.92)分,两组比较差异有统计学意义(t=2.645,P<0.05),观察组疗效优于对照组。TCD超声检查提示两组患者治疗后动脉Vm值均提高,但观察组疗效明显优于对照组,其差异有统计学意义(t=3.288,t=2.791;P<0.05)。结论:前列地尔注射液可有效缓解PCIV症状,TCD超声可作为病情观察和判断疗效的理想手段。

Objective To explore therapeutic effect of alprostadil for posterior circulation ischemic vertigo （PCIV） and the use value of transcranial Doppler （TCD） ultrasound. Methods： 90 patients with PCIV were selected in this research and all of them were divided into observation group （45 cases） and control group （45 cases）. The patients of control group received basic treatment and ones of observation group received alprostadil injection on the basic treatment. The improvement degrees of dizziness symptom pre and post treatment of them were scored. And the Vm of vertebral artery （VA） and basilar artery （BA） were measured and the differences of the two groups between pre and post treatment were compared. Results： The symptom score （0.27±0.54） of dizziness of observation group post treatment （0.27±0.54） was significantly lower than that of control group （0.69±0.92） （t=2.645, P〈0.05）, and this results meant observation group was better than that of control group. And the results of TCD ultrasound indicated that the Vm values of artery both of two groups were enhanced post treatment, while the curative effect of observation group was significantly higher than that of control group （t=3.288, t=2.791, P〈0.05）. Conclusion： Alprostadil injection can effectively relieve symptom of PCIV, and TCD ultrasound can be used as a hopeful method in observing disease and judging curative effect.
