脑卒中后认知障碍护理门诊的建立及实践

Construction and implementation of nursing clinic for post-stroke cognitive impairment

为规范脑卒中后认知障碍患者的管理,由精神卫生专科护士和老年专科护士组成了护理团队,构建了脑卒中后认知障碍护理门诊,为脑卒中患者提供综合管理。精神卫生专科护士负责建立患者认知管理档案、进行脑卒中患者的认知功能评估、实施认知康复训练及随访管理;老年专科护士负责脑卒中危险因素的防控指导、居家护理指导、组织开展健康教育讲座及社区科普活动等。脑卒中后认知障碍护理门诊实现了脑卒中后认知障碍患者的整体管理,满足了患者的个体需求,提高了患者及家属的满意度。

For the purpose of establishing the standard management process for patients with post-stroke cognitive impairment （PSCI）,our hospital established PSCI nursing clinic,and a nursing team including a mental health specialist nurse and a geriatric specialist nurse took charge of the clinic work. The mental health specialist nurse was responsible for establishment of patient document,assessment,rehabilitation exercise and follow-up. The geriatric specialist nurse was responsible for prevention and control of risk factors of stroke,familial nursing care guidance, health education lecture,and community popular science. The implementation of nursing clinic achieved compre-hensive management of PSCI, satisfied individualized needs of patients, and the nursing clinic has been approved by patients and their families.
