2型糖尿病患者中医延续护理方案的制订及实践

Establishment and practice of the traditional Chinese medicine continuation nursing program for patients with type 2 diabetes

目的制订中医延续护理方案,探讨其在2型糖尿病患者中的应用效果。方法组建中医延续护理团队,制订2型糖尿病患者中医延续护理方案内容,包括辨证施护方案、中医运动处方、情志护理等,实施中医延续护理方案,并探讨方案对2型糖尿病患者自我管理效能和糖化血红蛋白含量的影响。结果干预6个月后患者的自我管理行为总分为(103.84±12.28)分,与患者出院时自我管理行为总分(90.68±14.58)分的比较,差异具有统计学意义(t=-6.349,P<0.01),干预6个月后患者糖化血红蛋白含量(6.38±1.67)%与出院时(9.16±1.08)%比较,差异具有统计学意义(t=34.28,P<0.01)。结论中医延续护理方案可提高2型糖尿病患者的自我管理水平,降低患者糖化血红蛋白含量。

Objective To establish the traditional Chinese medicine continuation nursing program for patients with type 2 diabetes and to explore its effects. Methods The traditional Chinese medicine continuation nursing group were built,and the program was developed. Self-management effectiveness and glycosylated hemoglobin of pa- tients with type 2 diabetes was analyzed. Results After 6 months of implementation,the total score of the patients＇ self-management was increased from（90.68±14.58） at discharge to（103.84±12.28）,and the difference was statistical significant （t=-6.349,P〈0.01）. Clycosylated hemoglobin after implementation was （6.38±1.67） which was lower than （9.16±1.08） at discharge,and the difference was statistically significant （t=34.28,P〈0.01）, Conclusion The traditional Chinese medicine continuation nursing program for patients with type 2 diabetes can improve self-manage- ment behavior of patients and reduce the level of glyeosylated hemoglobin.
