全身麻醉对焦虑性抑郁症患者焦虑水平影响的临床观察

Observation on the Clinical Effect of General Anesthesia on the Anxiety Level of Anxiety Depression Patient

目的探究全身麻醉对焦虑性抑郁症患者焦虑水平的影响。方法选取2011年2月—2018年2月到我院麻醉科行全麻手术并符合纳入标准的98例患者纳入研究。采用严格的对照匹配方法,将所有患者分为焦虑性抑郁症组与对照组,每组各49例。两组均采用全身麻醉下手术治疗。对比两组患者术前12 h及术后24 h、72 h汉密尔顿焦虑量表(HAMA)评分差异,评价两组患者的焦虑情况。结果术前12 h,两组患者术后24 h、72 h HAMA评分比较,差异均具有统计学意义(P <0.01);对照组患者术前12 h HAMA评分均高于术后24 h、72 h的HAMA评分[术前12 h:(8.96±2.74)分,术后24 h:(8.15±3.17)分,术后72 h:(5.38±1.87)分],差异具有统计学意义(P<0.05);焦虑性抑郁症组患者术前与术后HAMA评分比较[术前12 h:(13.47±4.22)分,术后24 h:(13.85±4.32)分,术后72 h:(12.29±3.56)分],差异无统计学意义(P> 0.05)。结论对于焦虑性抑郁症患者来说全麻手术可能并不会影响患者的焦虑情况。

Objective To investigate the effect of general anesthesia on the anxiety level of anxiety depression patient. Methods 98 patients who had undergone general anesthesia from February 2011 to February 2018 and met the inclusion criteria were enrolled in the study. All patients were divided into anxiety depression group and control group with 49 cases in each group by strict matching method. The two groups were treated by general anesthesia. The difference of Hamilton Anxiety Scale （HAMA） scores between the two groups at 12 hours before operation and 24 hours and 72 hours after operation was compared, and the anxiety status of the two groups was evaluated. Results There were statistically significant difference for HAMA scoring between two groups at 12 h before operation and 24 h and 72 h after operation （P 〈 0.01）. The HAMA scores of 12 hours before operation in the control group were higher than those of 24 hours and 72 hours after operation [12 hours before operation： （8.96±2.74） points, 24 hours after operation： （8.15±3.17） points, 72 hours after operation： （5.38±1.87） points], the difference was statistically signifcant （P 〈 0.05）. There was no signifcant difference in HAMA score between pre-operation and post-operation in patients with anxiety depression [12 hours before operation： （13.47±4.22） points, 24 hours after operation （13.85±4.32） points and 72 hours after operation （12.29±3.56） points]. The difference was not statistically signifcant （P 〉 0.05）. Conclusion For the Anxiety Depression patients the operations under General Anesthesia might not have effect on the anxiety level.
