运动干预对衰弱老人跌倒、步态和平衡能力影响的Meta分析

Effects of exercise interventions on falls,gait and balance in frailty older people:a meta-analysis

[目的]评价运动干预对衰弱老人跌倒、步态能力和平衡能力的影响。[方法]全面检索PubMed、JBI、MEDLINE、OVID、Embase、中国生物医学文献数据库、中国知网、维普网、万方数据知识服务平台等数据库中关于运动干预对衰弱老人跌倒、步态能力和平衡能力影响的文献,经2名研究者按纳入与排除标准独立筛选文献、提取资料、评价文献质量,采用Revmen 5.3软件分别对运动干预对衰弱老人跌倒、步态能力和平衡能力的影响进行Meta分析。[结果]共纳入9项随机对照试验。Meta分析结果显示:运动干预后衰弱老人的跌倒发生率降低[OR=0.53,95%CI(0.37,0.78),P<0.05],快速步态能力改善[MD=0.20,95%CI(0.03,0.37),P<0.05],常速步态能力及平衡能力均有改善,肌肉力量改善[SMD=0.18,95%CI(0.02,0.35),P<0.05]。[结论]现有证据显示,运动干预可有效改善衰弱老人的步态能力和平衡能力、降低跌倒风险。

Objective：To evaluate the effectc of exercise intervention on fall in gait ability and balance in frailty older people. Methods： We searched PubMed, J BI, MEDI,INE, OVID, CBMdisc, CNKI, Embase, Wanfang Data, VIP to collect randomized controlled trials（RCTs） about effects of exercise interventions on falls,guit and hal ance in frailty older people. The quality of literatures was critically selected and evaluated and data were extrac ted by 3 reviewers independently. Meta analysis was conducted for the eligible RCTs. Then RevManS. 3 was used for Meta analysis. Results： Nine RCTs were included in this meta analysis. The results of meta analysis showed that,demonstrated decreased fall in failty old people[OR= 0. 53,95%CI（0.37,0.78）,P〈0.05]. En hancements in the rapid gait ability[MD=0. 20,95%CI（0.03,0.37）,P〈0.05]. The effects of exercise inter vention on the balance and strength performance[SMD=0.18,95%CI（0.02,0.35）,P〈0.05]. Conclusions：Ex ercise intervention was effective in improving gait ability, balance and reducing falls risk.
