椎体成形术与椎体后凸成形术治疗老年骨质疏松性单椎体压缩骨折的疗效比较

Percutaneous vertebroplasty versus percutaneous kyphoplasty in the treatment of elderly patients with osteoporotic vertebral compression fractures

目的比较椎体成形术(PVP)与椎体后凸成形术(PKP)治疗骨质疏松性压缩骨折的临床效果。方法 283例骨质疏松性单椎体压缩性骨折患者按手术方式分成PVP组(n=192)与PKP组(n=91)。比较2组的手术时间、住院时间、住院费用、疼痛评分、后凸角和受损椎体高度及再骨折发生率。结果 2组手术时间、住院费用比较有显著差异(P <0. 05)。2组患者术后3 d、1个月、3个月VAS评分较术前显著降低(P <0. 05)。2组椎体高度及后凸角恢复情况比较,差异有统计学意义(P <0. 05)。2组患者骨水泥渗漏及再骨折发生率比较,差异无统计学意义(P> 0. 05)。结论对于脊柱单椎体骨质疏松性压缩骨折患者,PVP术的疗效更为满意。

Objective To compare the effect of percutaneous vertebroplasty （PVP） and percutaneous kyphoplasty （PKP） in the treatment of patients with osteoporotic vertebral compression fractures. Methods A total of 283 patients with osteoporotic vertebral compression fractures were divided into PVP group （ n =192） and PKP group （ n =91） according to the type of surgery. The operation time, hospitalization time, hospitalization cost, pain score, kyphosis angle, height of injured vertebral body and incidence rate of refracture were compared between the two groups. Results There were significant differences in operation time and hospitalization cost between the two groups （ P 〈 0.05）. The VAS scores significantly decreased at 3 days, 1 months, and 3 months after operation in both groups （ P 〈0.05）. There were significant differences in the recovery of vertebral height and kyphosis between the two groups （ P 〈0.05）. There were no significant differences in the incidence rate of bone cement leakage and refracture between the two groups （ P 〉0.05）. Conclusion For patients with osteoporotic vertebral compression fractures of the vertebral column, efficiency of PVP is more satisfactory.
