不同手术方式治疗老年人股骨粗隆间骨折的疗效分析 被引量:6

Analysis of the Curative Effect of Different Surgical Methods for the Treatment of Intertrochanteric Fracture of the Femur in the Elderly

摘要 目的:探讨不同手术方式治疗老年人股骨粗隆间骨折的临床效果。方法:选择2015年1月-2018年1月本院收治的110例股骨粗隆间骨折老年患者110例,按照随机数字表法分为对照组和观察组,各55例。观察组采用防旋型股骨近端髓内钉(proximal femoral nail antirotation,PFNA)治疗,对照组采用动力髋螺钉(dynamic hip screw,DHS)治疗,对比分析两组手术及术后恢复指标、并发症情况,同时评估两组治疗6个月后髋关节功能。结果:观察组术中出血量少于对照组,手术时间与骨性愈合时间均短于对照组,差异均有统计学意义(P<0.05)。观察组患者术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。观察组髋关节功能优良率为92.73%,显著高于对照组的78.18%,差异有统计学意义(P<0.05)。结论:采用防旋型股骨近端髓内钉治疗老年人股骨粗隆间骨折的效果更为理想,临床应用价值较高,值得推广借鉴。 ObjectiveTo explore the clinical effect of different operative methods in the treatment of intertrochanteric fractures in the elderly.Method:110 elderly patients with intertrochanteric fracture in our hospital from January 2015 to January 2018 were selected,according to the random number table method,they were randomly divided into control group and observation group,55 cases in each group.The observation group was treated with proximal femoral nail antirotation(PFNA),the control group was treated with dynamic hip screw(DHS).The recovery index and complications of the two groups were compared and analyzed,the hip joint function of the two groups was evaluated after 6 months of treatment.Result:Intraoperative blood loss in the observation group was less than that in the control group,and operative time and bone healing time were shorter than those in the control group,the differences were statistically significant(P〈0.05).The incidence of postoperative complications in the observation group was lower than that in the control group,the difference was statistically significant(P〈0.05).The optimal rate of hip joint function was 92.73% in the observation group,significantly higher than 78.18% in the control group,the difference was statistically significant(P〈0.05).Conclusion:The effect of treatment of intertrochanteric fracture of the femur in the elderly with anti-rotation proximal nail is more ideal,the clinical application value is higher,which is worth using for reference.
