两种体表心电图方法判断急性下壁心肌梗死罪犯血管的比较 被引量:1

摘要 目的分析比较Fiol法和右冠状动脉积分法判断急性下壁心肌梗死(IWMI)罪犯血管的预测价值。方法收集2015年4月至2017年2月该院经心电图检查首诊为IWMI并行急诊冠脉造影确诊患者72例,分别以Fiol法和右冠状动脉积分法对罪犯血管进行判断,并以冠脉造影为判断罪犯血管"金标准",对比分析2种方法的准确率、灵敏度、特异度、阳性预测值和阴性预测值。结果 Fiol法判断IWMI的准确率高于右冠状动脉积分法,但差异无统计学意义(P>0.05)。在判断右冠状动脉病变为罪犯血管时,Fiol法灵敏度高于右冠状动脉积分法,差异有统计学意义(P<0.05)。结论 Fiol法和右冠状动脉积分法判断IWMI罪犯血管都有较高准确率,其中Fiol法相对简单,容易掌握,更适合在临床推广。 Objective To analyze and compare the predictive value between the Foil method and right coronary integTal method for judging the culprit vessels in acute inferior wall myocardial infarction.Methods 72 cases of acute IWMI initially diagnosed by EKG and definitely diagnosed by emergency coronary ang%gTaphy in the hospital from April 2015 to February 2017 were collected.The Fiol method and right coronary integral method were adopted to judge the culprit vessels.The coronary angiography served as the gold standard for judging the culprit vessels.The accuracy rate, sensitivity, specificity, positive predictive value and negative predictive value were compared between the two methods and analyzed. Results The accuracy rate of the Fiol method was higher than that of the right coronary integral method, but the difference was not statistically significant (P〉0.05).In judging the right coronary arteria as theulprit vessels, the sensitive of the Fiol method was higher than that of the right coronalT integral method, and the difference was statistically significant (P〈0.05). Conclusion The Fiol method and right coronary integral method have high accuracy in judging the culprit vessels of inferior wall myocardial infarction, in which the Fiol method is relatively more simple and easier to master, and is more suitable for clinical promotion.
