中西医结合SOD技术在急诊ICU的应用研究

Application of Integrated Traditional Chinese and Western Medicine SOD Technology in Emergency ICU

目的:研究中西医结合口咽脱污(SOD)技术在急诊重症加强护理病房(ICU)的应用。方法:选取2017年1月-2018年7月期间本院急诊ICU建立人工气道的患者120例作为研究对象,按照住院床位的不同将其分为试验组(1～5床)和对照组(6～10床),各60例。对照组给予传统的口腔护理,试验组给予中西医结合的口咽脱污技术法进行口腔护理,比较分析两组护理后的口腔不良、呼吸性相关性肺炎(VAP)和口咽拭子培养阳性发生情况。结果:试验组护理后口腔不良情况发生率为13.33%明显低于对照组的43.33%,差异有统计学意义(x^2=6.648,P=0.010)。试验组护理后VAP发生率为16.67%明显低于对照组的43.33%,差异有统计学意义(x^2=5.079,P=0.024)。试验组护理第3、7天的口咽拭子培养阳性率分别为23.33%和6.67%均明显低于对照组的70.00%和36.67%,差异均有统计学意义(x^2=13.125、7.954,P=0.000、0.005)。结论:在急诊ICU建立人工气道的患者中应用中西医结合SOD技术进行口腔护理,能够明显降低口腔不良情况发生率、VAP发生率和咽拭子培养阳性率,具有极高的应用价值。

Objective：To study the application of integrated traditional Chinese and Western medicine oraland pharyngeal decontamination（ SOD） technology in emergency intensive care unit（ ICU）.Method：A total of120 patients with artificial airway established in emergency ICU of our hospital from January 2017 to July 2018 wereselected as the study objects.According to the different beds,they were divided into experimental group（ 1-5 beds）and control group（ 6-10 beds）,60 cases in each group.The control group was given traditional oral nursing,theexperimental group was given oral and pharyngeal decontamination technology of integrated traditional Chinese andWestern medicine oral nursing.The occurrence of adverse oral conditions,respiratory associated pneumonia（ VAP）and positive rate of throat swab culture after nursing between two groups were compared and analyzed.Result：Afternursing,the incidence of adverse oral conditions in the experimental group was 13.33%,which was significantlylower than 43.33% in the control group,the difference was statistically significant（ X2=6.648,P=0.010）.After nursing,the incidence of VAP in the experimental group was 16.67%,which was significantly lower than 43.33% in the control group,the difference was statistically significant（ X2=5.079,P=0.024）.The positive rate of throat swab culture in the experimental group were 23.33% and 6.67% respectively on the third and seventhday of nursing respectively,which were significantly lower than 70.00% and 36.67% in the control group,thedifferences were statistically significant（ X2=13.125,7.954,P=0.000,0.005）.Conclusion：The application ofintegrated traditional Chinese and Western medicine SOD technology in oral nursing of patients with artificial airway in emergency ICU can significantly reduce the incidence of adverse oral conditions,VAP and positive rate of throatswab culture,and has a very high application value.
