负压吸引管及宫颈钳在经腹小切口输卵管结扎术中的应用观察

Application of Negative Pressure Suction Tube and Cervical Neoadler in Transabdominal Incisional Tubal Ligation

目的:探讨负压吸引管及宫颈钳在经腹小切口输卵管结扎术中的应用效果。方法:选取178例有女性绝育适应症的妇女为研究对象,采用随机数字表法将患者分为观察组和对照组,每组89例。观察组用负压吸引管及宫颈钳组成自制简易举宫器行局麻下经腹小切口输卵管结扎术。对照组采用传统局麻下经腹小切口输卵管结扎术,不用负压吸引管及宫颈钳组成的自制简易举宫器。统计手术时间、术中出血量、手术的成功率、术后下床活动的时间、术后住院的天数和术后并发症发生情况。结果:观察组手术时间、术中出血量显著低于对照组(P<0.05),两组术后下床活动的时间和术后住院的天数比较差异无统计学意义(P>0.05)。观察组和对照组手术成功率分别为100.00%和89.89%,两组比较存在显著性差异(P<0.05)。观察组和对照组并发症发生率分别为3.37%和8.99%,两组比较存在显著性差异(P<0.05)。结论:自制举宫器有效解决局麻下经腹小切口输卵管结扎术取管困难问题,提高手术成功率,减少术中出血,缩短手术时间,适用于基层医院的节育手术。

Objective： To evaluate the effect of negative pressure suction tube and cervical clamp in transabdominal incisional tubal ligation. Methods： A total of 178 women with female sterility indications were selected as the subjects. The patients were divided into observation group and control group by random number table. The observation group was treated with negative pressure suction tube and cervical clamp. The control group was treated with traditional local anesthesia with small incisional tubal ligation,without negative pressure suction tube and cervical clamp composed of self-made simple palace. The time of operation,the time of postoperative hospitalization,the number of days of postoperative hospitalization and postoperative complications were recorded. Results： The operative time and intraoperative blood loss were significantly lower in the observation group than in the control group（ P〈0.05）. There was no significant difference between the two groups in the time of postoperative out of bed and the number of days after hospitalization（ P〈0.05）. The success rates of the observation group and the control group were 100.00% and 89.89% respectively. There was significant difference between the two groups（ P〈0.05）. The incidence of complications in the observation group and the control group was 3.37% and 8.99%,respectively. There was significant difference between the two groups（ P〈0.05）. Conclusion： The self-made uterus can effectively solve the problem of difficulty in removing the tubal ligation of the local anesthesia with abdominal incision and tubal ligation,improve the success rate of operation,reduce intraoperative bleeding and shorten the operation time,and apply to the birth control of primary hospital.
