乳腺三维断层成像在全数字化乳腺钼靶X线摄影阴性病变中的应用价值 被引量:20

Application value of breast tomosynthesis in the negative lesions under full-field digital mammography

摘要 目的 探讨乳腺三维断层成像(DBT)对全数字化乳腺钼靶X线摄影(FFDM)阴性致密型乳腺病变的诊断价值.方法 回顾性分析山西省人民医院2017年9月至2018年6月312例乳腺病变患者的钼靶图像,FFDM结合DBT的临床诊断依据美国放射学会推荐的乳腺影像报告和数据系统(BI-RADS)并经手术病理证实.结果 312例乳腺病变患者中23例FFDM阴性而DBT阳性,其中15例为结构扭曲,8例为结节性病变;11例为乳腺癌(8例浸润性导管癌,3例浸润性小叶癌),其余12例中脂肪坏死2例,肉芽肿性炎症2例,硬化型腺病4例,导管上皮非典型增生2例,纤维瘤2例.结论 DBT可发现致密型腺体中常规FFDM不能发现的结节,尤其是致密型腺体重叠掩盖的FFDM不能发现的结构扭曲,DBT与FFDM结合可以提高乳腺癌的诊断率及早期发现率.
