股前外侧穿支皮瓣与腹部带蒂皮瓣修复术治疗手外伤组织缺损的疗效观察

Effects of anterolateral femoral perforator flap and abdominal pedicle flap repair in treatment of tissue defect of hand trauma

目的:探讨股前外侧穿支皮瓣与腹部带蒂皮瓣修复术治疗手外伤组织缺损的效果。方法:选择某院收治的128例手外伤组织缺损患者,将其应用随机数表法分为2组,每组各64例。对照组实施腹部带蒂皮瓣修复术,观察组行股前外侧穿支皮瓣修复术。观察2组修复效果、创面愈合时间、压觉及并发症等。结果:观察组肢体恢复优良率(92. 19%)、满意度(95. 31%)优于对照组的优良率(68. 75%)及满意度(78. 13%),差异有统计学意义(P <0. 05);较对照组创面愈合时间(22. 47±4. 60)d、压觉(0. 89±0. 24) g相比,观察组创面愈合时间(7. 15±1. 81) d、压觉(0. 37±0. 10) g相对较少,差异有统计学意义(P <0. 05); 2组患者并发症发生率比较,差异无统计学意义(P> 0. 05)。结论:股前外侧穿支皮瓣修复术为手外伤组织缺损治疗中较为有效且安全的术式,术后并发症少,利于恢复手部功能及外形,提升患者治疗满意度。

Objective ： To observe the effects of anterolateral femoral perforator flap and abdominal pedicle flap repair in treatment of tissue defect of hand trauma. Methods ： 128 patients with tissue defect of hand trauma treated in a hospital were selected and randomly divided into two groups, 64 cases in each group. The control group underwent abdominal pediele flap repair, while the observation group was treated with anterolateral femoral perforator flap repair. The repair effect, wound healing time, pressure sense, and complications of the two groups were observed. Results： The excellent reeovery rates of limbs and satisfaction in the observation group （92. 19%, 95.31% ） were higher than those of the control group （68.75%, 78.13% ） （ P 〈 0.05 ） ; the wound healing time and pressure sense in the observation group [ （7.15 ± 1.81）d, （0.37 ±0.10）g] were less than those of the control group [ （22.47± 4.60） d, （0.89 ±0.24）g] （P 〈0.05）. There was no statistical difference in the incidence of complications between the control group and the observation group （ P 〉 0.05 ）. Conclusion ： Anterolateral femoral perforator flap repair in treatment of tissue defect of hand trauma is effective and safe. It has less postoperative complications, and it can recover the function and shape of the affected hand and improve patients＇ treatment satisfaction.
