宫颈上皮内瘤变组织中高危型HPV感染的研究 被引量:9

摘要 目的分析宫颈上皮内瘤变(CIN)组织中高危人乳头瘤病毒(HPV)感染的分布状况及其临床意义。方法采用荧光定量基因扩增技术对205例CIN组织标本进行15种高危型HPV分型检测,其中CINⅠ级72例、CINⅡ级55例、CINⅢ级78例,分析不同级别CIN患者HPV感染率及HPV感染型别的差异。结果 205例CIN患者中检出HPV感染者165例,总HPV感染率为80. 5%(165/205),其中CINⅠ级HPV感染率为51. 4%(37/72)、CINⅡ级HPV感染率为94. 5%(52/55)、CINⅢ级HPV感染率为97. 4%(76/78)。在CINⅢ级患者中,HPV 16型感染出现频率高达41. 8%(46/110),随后是52型、58型、33型和31型。不同年龄CINⅠ级、CINⅡ级、CINⅢ级患者HPV感染率比较,差异有统计学意义(P<0. 05)。结论随着CIN病变程度的增加,高危型HPV16型感染率也随之增加。对适龄女性行宫颈高危型HPV分型检测,有利于临床妇产科医师对CIN进行诊断、治疗和随访,也有利于临床医师跟踪和随访高致癌型HPV感染者。 Objective To compare the distribution states of 15 kinds of high-risk human papillomavirus genetypes in tissues of cervical intraepithefial neoplasia (CIN) and its clinical significance. Methods Polymerase chain reaction (PCR) and fluorescence quantitative technology were utilized for the detection of 15 kinds of HR-HPV genetypes in tissue specimens from 205 cases of cervical intraepithelial neo- plasia, including 72 cases of CIN I grade, 55 cases of CINⅡ grade and 78 cases of CIN Ⅲ grade. And related materials of all subjects were analyzed. Results In 205 cases of cervical intraepithelial neoplasia, total infection rate of HPV was 80. 5% ( 165/205 ) , the infection rate of HPV was 51.5% (37/72) in CIN I grades, the infection rate of HPV was 94. 5% (52/55) in CIN Ⅱ grades and the infection rate of HPV was 97.4% ( 76/78 ) in CIN m grades. Conclusion Along with the increase of the cervical lesion degree, the infection rate of HR-HPV 16 type is gradually increased in cervical precancerous lesions, and present frequency of 16 type infection is 41.82% (46/110) in CINⅢ grades, following types are 52, 58, 33 and 31. HR-HPV typing detection will benefit clinical gynecologist to diagnose, treat and follow-up cervical precancerous lesion patients. The detection also benefit clinical physician on determining high-carcinogenic HPV infected person and conduct tracking follow-up to the patients.
