B-Lynch缝合术在高危妊娠剖宫产并发产后出血中的效果分析

Analysis of the effect of B-Lynch suture in postpartum hemorrhage of cesarean section in high risk pregnancy

目的探讨B-Lynch缝合术在防治高危妊娠剖宫产患者并发产后出血中的应用效果,为临床提供参考。方法选择2014年1月-2016年12月绵阳四〇四医院收治的120例高危妊娠剖宫产患者为研究对象,所有对象分为观察组和对照组,每组各60例,对照组给予缩宫素注射+米索前列醇+常规缝合,观察组给予缩宫素注射+米索前列醇+B-Lynch缝合,对比两组患者产后出血发生率,同时观察B-Lynch缝合对产妇预后效果的影响。结果观察组产后出血发生率为46. 7%,对照组为76. 7%,观察组产后2 h、24 h出血量均明显少于对照组(P<0. 05),住院时间明显短于对照组(P<0. 05),24 h血红蛋白水平明显高于对照组(P<0. 05)。观察组成功止血率高于对照组(P<0. 05)。结论 B-Lynch缝合对高危妊娠剖宫产患者并发产后出血有良好的防治效果,能够减少出血量,改善产妇血红蛋白水平,缩短住院时间。

Objective To investigate the effect of B-Lynch suture in postpartum hemorrhage of cesarean section in high risk pregnan- cy. Methods 120 cases of high-risk pregnancy cesarean section complicated postpartum hemorrhage were divided into 2 groups, control group （ 60 cases） was given oxytocin injection and misoprostol, conventional suture; observation group （60 cases） was given oxytocin in- jection, and misoprostol, B- Lynch suture. The incidence of postpartum hemorrhage, and the effect of B- Lynch suture were compared. Results In observation group, the postpartum hemorrhage rate （46. 7% ） was significantly lower than that of control group （76. 7% ）, the amount of postpartum bleeding at 2 h, 24 h were significantly less than those of control group （P〈 0. 05）, hospitalization time （5.6± 1 ） d was significantly shorter than that of control group （ 7.5 ± 1.2 ） d （ P〈 0. 05 ） ; at 24 h, the hemoglobin level （ 92.4 ± 6. 7 ） g/L was significantly higher than that of the control group （81.6±5.8） g/L （P〈 0. 05 ） . In observation group, the successful hemo- stasis rate （ 100% ） was higher than that in control group （91.7%, P〈 0. 05 ） . Conclusion B-Lynch suture for high-risk pregnancy ce- sarean section postpartum hemorrhage in patients with postpartum hemorrhage has a good control effect, it can reduce the amount of bleed- ing, improve maternal hemoglobin level, shorten the length of stay.
