前哨淋巴结活检在早期浸润性乳腺癌保乳手术中的应用

Aplication of sentinel lymph node biopsy in diagnosis of early invasive breast cancer

目的分析前哨淋巴结活检术(SLNB)对早期浸润性乳腺癌腋窝淋巴结转移的诊断效能及手术中的应用价值。方法回顾性分析我院乳腺外科2012年7月至2015年5月收治的217例早期浸润性乳腺癌患者的临床资料。应用SLNB对腋窝淋巴结转移的诊断效能,将SLN阴性者根据患者意愿分为2组,清扫腋窝淋巴结者为对照组,未清扫者为观察组,对比其手术情况、术后转归及生存质量。结果 SLNB诊断腋窝淋巴结转移的诊断效能均较高。观察组患者手术情况与对照组相比差异有统计学意义(P <0. 05),观察组并发症发生率低于对照组(P <0. 05);治疗后,2组患者功能和症状评分、总生存质量评分差异均有统计学意义(P <0. 05)。结论 SLNB对早期浸润性乳腺癌腋窝淋巴结转移的诊断效能较高,用以指导保乳手术可降低术后并发症、提高患者的生存质量,具有较好的应用价值。

Objective To evaluate the diagnostic value of sentinel lymph node biopsy（SLNB） in the diagnosis of axillary lymph node metastasis of early invasive breast cancer and its application value. Methods The diagnostic efficacy of SLNB in axillary lymph node metastasis was analyzed retrospectively in 217 patients with early invasive breast cancer who admitted to our breast surgery department from July2012 to May 2015. SLN negative patients were divided into two groups according to the patient’s intention,the axillary lymph nodes were cleaned as the control group,the unclean sweepers were the observation group,compared the operation condition,the postoperative outcome and the quality of life between two groups. Results The diagnostic efficacy of SLNB in the diagnosis of axillary lymph node metastasis was high. There was significant difference in the operation condition between two groups（P 〈 0. 05）. The incidence of complications in the observation group was lower than that in the control group（P 〈 0. 05）. After treatment,the scores of the functional and symptom scores and the total quality of life were significantly different between the two groups（P 〈 0. 05）. Conclusion SLNB can be used to diagnose axillary lymph node metastasis of early invasive breast cancer,and to guide breast conserving surgery can reduce the postoperative complications and improve the quality of life of patients. It has a good application value.
