A qualitative study about clinical nurses' annoyance at work

摘要 目的探讨临床护士的工作困扰及采取的应对措施,为改进护理服务与管理提供参考。方法采用便利取样法纳入64名护士撰写的临床工作中面临的困扰及采取的应对措施文本,采用Colaizzi现象学方法及Nvivo 8.0软件进行资料分析。结果临床护士的工作困扰可归纳为三个主题,分别是工作内容(包括涉及护理实务、教学、管理和职业发展)、人际关系(包括护患关系和医护关系)及个人价值,其中最普遍存在的困扰依次为护患关系(48.44%,31/64)、护理实务(42.19%,27/64)和个人价值(29.69%,19/64),应对措施反映出个体、医院和社会三个层面的努力。结论临床护士面临诸多工作困扰,管理者及护士自身均应做出积极应对,以提高护士队伍的工作质量和生活品质,促进优质护理服务的可持续发展。 Objective To explore nurses' annoyance at work and their coping measures to inspire the improvement of nursing service and management. Methods The convenience sampling method was adopted for the selection of texts written by 64 nurses about their annoyance and coping. The data was analyzed using the Colaizzi's phenomenological method with the aid of Nvivo 8.0. Results Three themes were identified about nurses' annoyance at work: job content( including nursing practice,teaching,management and professional development),interpersonal relationship( mainly including nurse-patient relationship and doctor-nurse relationship) and personal values. The top three common annoyances were nurse-patient relationship( 48.44%,31/64),nursing practice( 42.19%,27/64) and personal value( 29.69%,19/64). The coping measures reflected the efforts made at three levels,individual,hospital and societal. Conclusions Clinical nurses faced a great deal of annoyance at work. Managers and nurses should cope positively to improve nurses' quality of work and life,facilitating the sustainable development of the high-quality nursing service.
