药品精细化用法用量规则在我院门诊处方前置审核系统中的开发与实践

The development and practice of the precise usage and dosage rules of drugs in the preapproval system of outpatient prescription in our hospital

目的开发设置处方前置审核系统精细化用法用量规则。方法根据药品说明书、国内外最新的用药指南和专家共识设置精细化用法用量,多方面完善规则。选取第二军医大学长征医院2016年12月～2017年8月门诊全处方数据,分析比较设置前后平均处方不合格率、用法不适宜及超剂量不合格处方比例。结果设置后门诊平均处方不合格率为19.17%,低于设置前门诊的27.83%(P=0.019)。与设置前比较,设置后用法不适宜不合格处方比例、超剂量不合格处方比例下降(P <0.05)。结论在医生开立处方时,前置处方审核系统运行精细化规则,实时拦截不合理处方,使门诊不合格处方率明显下降。

Objective To develop and set precise usage and dosage rules for prescription pretrial system. Methods The precise usage and dosage were set based on the description of the medicine, the latest domestic and international instruction of prescription, and the consensus of experts, in order to complete the rules from multiple aspects. The all prescription date were selected from Changzheng Hospital, the Second Military Medical University from December 2016 to August 2017, and the average ratio of disqualified, unsuitable usage and overdosed defective prescription was analyzed. Results The average ratio of defective prescription was 19.17%, which was lower than the ratio （27.83%） before the setting the system （P = 0.019）. The ratio of unsuitable usage and overdosed defective prescription decreased （P 〈 0.05）. Conclusion When a doctor prescribes a prescription, the pre-prescription review system runs refined rules to intercept unreasonable prescription in time, so that the ratio of unqualified prescription in outpatient decreases significantly.
