双柏散外敷治疗轻型急性胰腺炎的疗效观察

Clinical Observation of External Application of Shuangbai Powder in Treating Mild Acute Pancreatitis

【目的】观察双柏散外敷治疗轻型急性胰腺炎(脾心痛)的临床疗效。【方法】将60例轻型急性胰腺炎患者随机分为双柏散组和对照组,每组各30例。对照组给予西医常规治疗,双柏散组给予双柏散外敷联合西医常规治疗,疗程7 d,观察2组腹痛和腹部压痛改善情况、肠鸣音和排气排便恢复情况,以及血白细胞和血、尿淀粉酶恢复正常时间,并评价2组临床疗效。【结果】(1)治疗7 d后,双柏散组总有效率为96.67%,对照组为80.00%,双柏散组疗效优于对照组(P<0.05)。(2)双柏散组的腹痛改善率、腹部压痛缓解率、肠鸣音和排气排便恢复率均明显优于对照组(P<0.05);生存分析Log-rank检验结果显示:双柏散组的腹痛改善时间、腹部压痛缓解时间、肠鸣音和排气排便恢复时间均明显短于对照组(P<0.05)。(3)双柏散组的血白细胞和血、尿淀粉酶恢复正常时间均明显短于对照组,差异均有统计学意义(P<0.05)。【结论】双柏散外敷治疗轻型急性胰腺炎(脾心痛)患者,疗效确切,能有效缓解患者的症状和体征并缩短病程。

Objective To observe the clinical efficacy of external application of Shuangbai Powder for the treatment of patients with mild acute pancreatitis（precordial pain due to spleen disorder）. Methods Sixty patients with mildacute pancreatitis were randomly divided into Shuangbai Powder group and control group, 30 cases in each group.The control group received routine western medicine treatment,and the Shuangbai Powder group received externalapplication of Shuangbai Powder combined with routine western medicine treatment. The treatment covered 7 days.The improvement of abdominal pain,abdominal tenderness,bowel sound frequency,recovery of passing gas anddefecation,and time for white blood cell as well as blood and urine amylase recovering normal was observed,andthe clinical efficacy in the two groups was evaluated. Results（1）After treatment for 7 days,the total effective ratewas 96.67% in Shuangbai Powder group,and was 80.00% in the control group,the difference being significant（P〈0.05）.（2）The improvement rate of abdominal pain, the relief rate of abdominal tenderness, and therecovery rate of bowel sound frequency,passing gas and defecation in Shuangbai Powder group were superior tothose in the control group（P〈0.05）. Log-rank test results showed that time for the improvement of abdominalpain,abdominal tenderness,bowel sound frequency,and passing gas and defecation was shortened as comparedwith that in the control group（P〈0.05）.（3）Time for white blood cell as well as blood and urine amylaserecovering normal in Shuangbai Powder group was significantly less than that in the control group,the differencebeing significant（P〈0.05）. Conclusion External application of Shuangbai Powder is effective for the treatment ofpatients with mild acute pancreatitis（precordial pain due to spleen disorder）by relieving the symptoms and signs and by shortening disease course.
