摘要
为探讨消化道肿瘤根治术后早期血浆白蛋白降低的相关因素,本研究选取自2013年11月至2017年6月我院收治的消化道肿瘤根治术患者共80例,依照消化道肿瘤根治术后的早期血浆白蛋白是否降低分为观察组(n=21例)和对照组(n=59例)。其中,对照组术后早期血浆白蛋白未降低,观察组术后早期血浆白蛋白降低,记录统计两组临床资料,检测血浆ABL、PA、BUN和细胞因子浓度水平;采用羟乙基淀粉法检测观察组和对照组血浆容积并进行单因素相关分析和多因素Logistic回归分析。单因素结果显示:消化道肿瘤根治术后早期血浆白蛋白水平与性别、年龄、TNF-α、IL—1β水平差异无统计学意义(p〉0.05);消化道肿瘤根治术后早期血浆白蛋白水平与手术时间、血浆ALB、PA、BUN和血浆容积水平关系密(p〈0.05);Logistic回归分析显示:消化道肿瘤根治术后早期血浆白蛋白水平与手术时间、血浆ALB、PA、BUN和血浆容积水平关系密切(p〈0.05)。由此推测,消化道肿瘤根治术后早期血浆白蛋白降低影响因素相对较多,应采取有效的措施进行干预,提高手术成功率。
In order to investigate the related factors of early decrease of plasma albumin after radical resection of digestive tract, 80 inpatients of our hospital from October 2013 to October 2016 were selected, who were underwent radical resection of gastrointestinal cancer, and they were divided into observation group (n=21 cases) and control group (n=59 cases) according to whether the early plasma albumin was decreased. In the control group, plasma albumin was not decreased in the early stage, and the plasma albumin was decreased in the observation group. The plasma ABL concentration, PA, BUN and cytokine levels were measured. The hydroxyethyl starch was used to detect the two groups plasma volume and single factor and multivariate logistic regression analysis. The results shown that there was no significant difference in plasma albumin level between gender age, TNF-oL and IL-113 after radical resection of digestive tract tumor (p〉0.05); early plasma after radical resection of digestive tract (p〈0.05). Logistic regression analysis showed that plasma albumin level and operation time, plasma ALB, PA, BUN and plasma levels were significantly correlated with the time of operation, plasma ALB, PA, BUN and plasma volume (p〈0.05), plasma volume level was closely related (p〈0.05). So we could come to all conclusion that there are relatively few factors influencing plasma albumin reduction in radical resection of digestive tract tumor,and effective measures should be taken to improve the success rate of operation.
