离体肾转运器在供肾保护功能评估的临床应用

摘要 目的探讨离体肾转运器(LifePort)在公民死亡器官捐献(donation after citizen’s death,DCD)供肾移植中的临床应用。方法选择我院2013年1月-2017年12月16例DCD供肾采用LifePort灌注保存,回顾性分析16例采用LifePort灌注保存供肾移植术后第1、7、21d血肌酐水平、移植肾功能恢复延迟(DGF)发生率、急性排斥反应(AR)发生率,受者平均住院时间。结果 LifePort灌注保存与传统静态冷保存法结果比较,供肾移植术后第1、7、21d血肌酐水平下降更明显(P<0.05);移植肾功能恢复延迟(DGF)发生率、急性排斥反应(AR)发生率明显降低(P<0.05);受者平均住院时间明显缩短(P<0.05,见表1)。结论 LifePort有助于改善DCD供肾质量,减少术后DGF、AR发生率,缩短患者住院时间,提高DCD肾移植的远期疗效。 Objective To explore the clinical application of LifePort in kidney transplantation of donation after citizen's death (DCD). Methods Choosing 16 cases of the patients DCD renal perfusion preservation used LifePort in the period from January 2013 to December 2017,The serum creatinine levels of 16 patients were retrospectively analyzed on the first,seventh and 21st days after LifePort perfusion for renal transplantation,incidence of delayed renal graft function (DGF),incidence of acute rejection (AR) and the average length of hospital stay. Results The results of LifePort perfusion preservation were compared with the traditional static cold preservation method,serum creatinine level decreased more significantly on the first,seventh and 21st days after renal transplantation (P〈0.05),incidence of delayed renal graft function (DGF),the incidence of acute rejection (AR) was significantly reduced (P〈0.05),The mean length of hospitalization was significantly shortened (P〈0.05,as shown in table 1). Conclusion LifePort helps to improve the kidney quality of the DCD supply,the incidence of postoperative DGF and AR was reduced,and the length of hospital stay was shortened,to improve the long-term effect of DCD kidney transplantation.
