侧角度照射技术结合三维超声成像在胎儿腭完整显示的应用价值 被引量:4

摘要 目的评估侧角度照射技术与三维超声成像技术联合应用对完整显示胎儿腭结构的价值。方法抽选208例到我科进行胎儿超声系统筛查的孕24-28周孕妇,运用双盲法,分为标准组104例(运用常规照射角度,经过三维重建得出的检查结果)与侧角度照射组104例(运用侧角度照射,经过三维重建得出的检查结果)进行胎儿超声系统筛查,统计胎儿腭不同部分的显示率、胎儿腭裂的产前检出率以及各分型的检出率。结果侧角度照射组原发腭、继发腭大部分(96例,92.3%)均可显示明显优于标准组大部分(89例,85.6%)只显示原发腭,侧角度照射组胎儿腭的结构显示图像质量明显高于标准组P<0.05;侧角度照射胎儿腭裂的检出率为100%明显高于常规照射角度检出率为64.6%(P<0.05),分型结果提示,侧角度照射在继发腭裂、软腭裂、完全性腭裂的检出率均高于常规照射角度组。结论侧角度照射技术与三维超声成像技术的结合应用,能完整、全面、直观地显示胎儿腭,准确诊断腭裂,提高唇腭裂的分型诊断,进一步明确累及范围。 Objective To evaluate the value of the combined application of lateral Angle irradiation and three-dimensional ultrasonography in the full display of fetal palatal structure. Methods selected 208 cases of fetal ultrasound screening system to our department at 24 to 28 weeks pregnant women,using the double blind method,divided into standard group 104 cases (the use of conventional irradiation Angle,through 3d reconstruction of inspection results) and side Angle exposure group, 104 cases (using irradiation side Angle,through 3 d reconstruction of inspection results) for screening fetal ultrasound system,statistical palatal rate of different parts of the fetus,fetal cleft palate of prenatal detection rate and the detection rate of each type. Results most of the primary and secondary palates (96 cases,92.3%) of the lateral angle-irradiated group were significantly superior to the majority (89 cases,85.6%) of the standard group,only showing the primary palates. The detection rate of fetal cleft palate was 100% higher than that of conventional irradiation (P〈0.05). The results indicated that the detection rate of secondary cleft palate,soft cleft palate and complete cleft palate were all higher than that of conventional irradiation. Conclusion the combination of lateral Angle irradiation and three-dimensional ultrasonography can fully,comprehensively and intuitively show fetal palates,accurately diagnose cleft palate, improve the classification of cleft palate and further clarify the extent of involvement.
