分级诊疗背景下山东、河南、四川三省农村居民基层首诊意愿调查——基于6766位农村居民的调查与研究

Surveying The Rural Residents' Willingness of Primary Diagnosis in Shandong,Henan and Sichuan Province under The Background of Graded Diagnosis and Treatment

目的:对山东、河南和四川三省农村居民基层首诊意愿进行分析,从政策客体主观意愿的角度,探讨在农村实现基层首诊的阻力程度,为更好地实施基层首诊政策提供参考。方法:采用描述性统计分析对样本进行简单描述,采用卡方检验对三省数据及不同特征的农村居民患不同疾病时的首诊意愿进行分析。结果:村卫生室、自我医疗和私人诊所是农村居民患小病时的主要选择,占83. 42%;农村居民患常见病时选择县医院首诊的比重最高,占43. 41%;市医院和县医院是农村居民看大病时的主要选择,占82. 72%;农村居民在患小病、常见病和大病时,因年龄、健康状况和经济水平不同,其首诊机构意愿存在明显区别,其差异有统计学意义。结论:小病不出村可实现,常见病不出乡较困难,大病不出县有难度。

Objective To analyze the rural residents＇ willingness of primary diagnosis n Shandong, Henan and Sichuan Province, study its resistance, and provide references for improving the implementation of grassroots first visit policy. Method The willingness of primary diagnosis of rural residents with different diseases was analyzed with descriptive statistical analysis and chi square test. Results Village clinics , self - treatment and private clinics were the main choices of rural residents suffering from ailments, which accounted for 83.42%. County hospital was the high -proportion choice of rural residents suffering from common diseases, which accounted for 43.41%. City hospital and county hospital was the main choices of rural residents, which accounted for 82.72%. The rural residents＇ willingness of primary diagnosis was significantly different among residents with different age, health status and economic Condition. Cgnclusion It can be concluded that ailments can be treated in village clinics, common diseases and serious diseases need to be treated in hospitals with higher grade.
