广西边境地区县级医院突发公共卫生事件应急能力综合评价

Comprehensive Evaluation of Public Health Emergency Response Capacities of County-level Hospitals in Border Areas of Guangxi

目的:建立县级医院突发公共卫生事件应对能力评价指标体系及综合评价模型,并据此进行实证评价。方法:通过文献研究和德尔菲专家咨询法,构建指标体系,据此设计问卷,对广西边境地区22所县级医院进行调查。采用因子分析法建立综合评价模型并进行实证评价。结果:评价指标体系包括应急指挥协调、应急预案、监测预警、信息报告与交流、应急人员、应急床位、相关物质储备等7个维度。调查问卷的各维度具有较好的内部一致性(克朗巴赫α系数为0. 9467)和较好的内容效度和结构效度。通过因子分析方法最终提取了4个公因子,建立应对突发公共卫生事件能力综合评估模型:F=0. 3013 F1+0. 2709 F2+0. 2142 F3+0. 2136 F4(F1为保障因子,F2为预警因子,F3为床位因子,F4为信息因子)。结论:县级医院应急能力评价指标体系具有合理性、科学性和较好的信度与效度,综合评价模型能较全面的评价县级医院应对突发公共卫生事件的能力。广西边境地区县级医院卫生应急能力需要进一步加强。

Objective To establish an evaluation index system and a comprehensive evaluation model for the public health emergency response capacity of .county - level hospitals. Methods The evaluation index system was constructed through the literature research and the Delphi export consultation method. According to the system, a questionnaire on response capacities of county - level hospital was designed. Questionnaire surveys were conducted among 22 county - level hospitals in the border areas of Guan- gxi. Comprehensive evaluation model was established by factor analysis method and empirical stuciy was conducted. Results The evaluation index system to public health emergency response capacities of county - level hospitals includes 7 dimensions, emergency command and coordination, emergency plan, monitoring and early warning capabilities, information reporting and communication, emergency personnel, emergency bed, emergency related substances reserve. Each dimension has good internal consistency （ Kro- ne Bach＇ s alpha coefficient was 0. 9467）, and the questionnaire had preferably content validity and construct validity. By using factor analysis method, four common factors were extracted, and comprehensive evaluation model was established for the county - level hospitals to deal with public health emergencies： F =0. 3013 F1 ＋ 0. 2709 F2 ＋ 0. 2142 Fz ＋ 0. 2136 F4（ F1 as the guaran- tee factor, F2 as the early warning factor, F3 as the factor of bed, F4 as the factor of information）. Conclusions The evaluation index system of the emergency response capability to county - level hospitals is reasonable, scientific, and has good reliability and validity. The model can comprehensively evaluate the capabilities of county - level hospital to deal with public health emergencies. The public health emergency response capacities of county - level hospitals in Guangxi border areas needs to be strengthen.
