来曲唑治疗男性少弱精子症的效果分析

Analysis on effects of letrozole in the treatment of male oligoasthenozoospermia

目的 探求男性少弱精子症采用芳香化酶抑制剂—来曲唑治疗对其疗效的影响.方法 选择2015年4月至2017年4月焦作市妇幼保健院收治的少弱精子症男性患者96例,根据盲抽法分两组,每组48例.对照组采用十一酸睾酮软胶囊治疗,观察组采用来曲唑治疗,1个疗程后,比较两组精液质量、生殖激素水平及不良反应发生率.结果 治疗后,观察组精子总数[（60.32±11.37）×10^6/ml]、精液浓度[（23.24±2.87）×10^6/ml]、精子存活率[（42.31±4.52）％]、黄体生成素[（10.38±2.52）mIU/ml]、卵泡刺激素[（9.03±2.85）mIU/ml]、睾酮[（11.25±2.04）ng/ml]水平高于对照组[（42.65±12.52）×10^6/ml、（18.26±3.02）×10^6/ml、（30.65±4.21）％、（6.26±2.03）mIU/ml、（7.88±2.29）mIU/ml、（8.95±2.33）ng/ml],雌二醇水平[（30.25±8.59）pg/ml]低于对照组[（36.26±9.52）pg/ml],差异有统计学意义（P〈0.05）;两组不良反应发生率比较差异未见统计学意义（P〉0.05）.结论 来曲唑可提高男性少弱精子症患者精液质量,改善生殖激素水平,不良反应较少,安全性较高.

Objective To investigate the efficacy of aromatase inhibitor-letrozole on male oligoasthenozoospermia . Methods Ninety-six patients with oligoasthenozoospermia hospitalized in Jiaozuo Maternal and Child Health-Care Hospital from April 2015 to April 2017 were selected , and they were divided into two groups by blind selection method , with 48 cases in each group .Patients in control group were given testosterone undecanoate soft capsules , while patients in observation group were treated with letrozole .After one course of treatment , the semen quality , reproductive hormone level , incidence of adverse reactions were compared between the two groups .Results After treatment , the total sperm number, sperm concentration, sperm survival rate and the levels of luteinizing hormone （LH）, follicle stimulating hormone （ FSH）, testosterone （ T） in observation group [（60.32 ±11.37） ×10^6/ml, （23.24 ±2.87） ×10^6/ml, （42.31 ±4.52）%, （10.38 ±2.52） mIU/ml, （9.03 ±2.85） mIU/ml, （11.25 ±2.04）ng/ml] were higher than those in control group [（42.65 ±12.52） ×10^6/ml, （18.26 ± 3.02） ×10^6/ml, （30.65 ±4.21）%, （6.26 ±2.03） mIU/ml, （7.88 ±2.29） mIU/ml, （8.95 ± 2.33）ng/ml], while the E2 level of observation group [（30.25 ±8.59）pg/ml] was lower than that of control group [（36.26 ±9.52）pg/ml], P〈0.05.There was no difference in the incidence of adverse reactions between the two groups （ P〉0.05 ） . Conclusions Letrozole in the treatment of male oligoasthenozoospermia can increase semen quality , improve reproductive hormone level , has less adverse reactions and higher safety .
