无偿献血者HTLV筛查模型的研究

A Study of HTLV screening model for voluntary blood donors

目的分析厦门地区无偿献血者HTLV感染特征,建立1种新型的经济有效的筛查模型。方法收集2014—2017年,厦门中心血站所有无偿献血者标本,用ELISA法进行抗-HTLV筛查,ELISA反应性标本用蛋白印迹法（WB）做确证实验,确定是否为真阳性。收集确证阳性献血者人口特征信息,分析其特征并建立新的筛查模型。结果 2014—2017年共筛查献血者标本201 362份,初筛反应性标本270份,4年的流行率分别为0. 21%、0. 07%、0. 14%和0. 13%。经WB确证实验检测,20份标本确证为抗-HTLV阳性,确证阳性率分别为0. 008 7%、0. 008 5%、0. 012%和0. 010%。20位确证抗-HTLV阳性献血者全部为首次献血者。结论根据研究结果,建立1个新的筛查模型,在常规筛查的地区,可以只对首次献血者筛查HTLV。

Objective To analyze the characteristics of HTLV infection in voluntary blood donors in Xiamen,and to establish a novel cost-effective screening model for the region. Methods All the voluntary blood donor specimens from Xiamen blood center were collected from 2014 to 2017. The anti-HTLV screening was performed by ELISA. The ELISA reactive specimens were confirmed by Western blotting（ WB） to determine final positivity. Collected demographic characteristics of confirmed positive donors,analyzed their features and then establish the new screening model. Results Between January 2014 and December 2017,a total of 201 362 blood donor specimens were screened,and 270 were reactive. The prevalence were0. 21%,0. 07%,0. 14%,and 0. 13%,respectively. After WB confirmatory test,20 specimens were confirmed to be antiHTLV positive with positive rates of 0. 008 7%,0. 008 5%,0. 012% and 0. 010%,respectively. The 20 anti-HTLV confirmed positive blood donors were all first blood donors. Conclusion Based on the results of this study,we established a novel screening model,in which HTLV can be screened only for first-time donors in routinely screened areas.
