全自动冷沉淀制备仪的制备质量和效率评价 被引量:14

摘要 目的评价全自动冷沉淀制备仪的制备质量和效率。方法将400袋新鲜冰冻血浆按照不同规格随机分为手工虹吸法组(简称手工组)和全自动冷沉淀制备仪组(简称自动组)。测定冷沉淀的Ⅷ因子和纤维蛋白原含量,对照质量要求计算符合率,采用SPSS12. 0做统计学分析。以每小时人均制备量为效率指标进行比较。结果Ⅷ因子含量均值的比较,3种规格(2. 0 U、1. 5 U和1. 0 U)自动组分别为(130. 64±16. 91) IU/袋、(103. 89±32. 44) IU/袋和(72. 35±18. 37) IU/袋,手工组分别为(87. 61±15. 33) IU/袋、(65. 43±11. 18) IU/袋和(49. 54±10. 24) IU/袋(均P〈0. 05);Ⅷ因子含量符合率(95%CI)的比较,3种规格自动组分别为99. 17%(97. 5%—100%)、100%(100%—100%)、100%(100%—100%),手工组分别为82. 50%(75. 6%—89. 4%)、85. 00%(73. 4%—96. 6%)、82. 50%(70. 2%—94. 8%)(均P〈0. 05)。纤维蛋白原含量均值的比较,3种规格自动组分别为(254. 26±73. 10) mg/袋、(204. 32±64. 54) mg/袋和(139. 44±30. 64) mg/袋,手工组分别为(184. 89±35. 58) mg/袋、(150. 42±35. 82) mg/袋和(120. 75±25. 24) mg/袋(均P〈0. 01);纤维蛋白原含量符合率的比较,3种规格自动组均为100%(100%—100%),手工组分别为92. 50%(87. 7%—97. 3%)、95. 00%(87%—91%)、92. 50%(84. 0%—100%),仅2. 0 U规格组的差异具统计学意义(P〈0. 05)。每小时人均制备量,3种规格自动组分别为19. 32袋、22. 47袋和28. 17袋,平均为21. 25袋,手工组分别为7. 95袋、10. 81袋和12. 82袋,平均为9. 12袋。结论全自动冷沉淀制备仪制备的冷沉淀的Ⅷ因子和纤维蛋白原含量及其符合率均显著高于手工虹吸法,制备效率高,节省人力,值得进一步推广应用。 Objective To evaluate the performance and working efficiency of a new type of cryoprecipitate preparation equipment( Auto Cryopreper). Methods 400 bags of FFP were divided randomly into Manual siphoning method group( Group Manu) or Auto Cryopreper group( Group Auto). The contents of factor Ⅷ and fibrinogen in cryoprecipitate were measured with automatic blood coagulation analyzer and analyzed statistically with SPSS. Results The average contents of Factor Ⅷ were respectively( 130. 64±16. 91) IU/bag,( 103. 89±32. 44) IU/bag and( 72. 35±18. 37) IU/bag in Group Auto( 2. 0 U,1. 5 U,1. 0 U) and( 87. 61±15. 33) IU/bag,( 65. 43±11. 18) IU/bag and( 49. 54±10. 24) IU/bag in Group Manu,with the difference being significant between two groups( P〈0. 05). The rates of conformance( 95%CI) to quality requirement for contents of Factor Ⅷ were respectively 99. 17%( 97. 5%—100%),100%( 100%—100%) and 100%( 100%—100%) in Group Auto( 2. 0 U,1. 5 U,1. 0 U) and 82. 50%( 75. 6%—89. 4%),85. 00%( 73. 4%—96. 6%) and 82. 50%( 70. 2%—94. 8%) in Group Manu,with the difference being significant between two groups( P〈0. 05). The average contents of fibrinogen were respectively( 254. 26±73. 10) mg/bag,( 204. 32±64. 54) mg/bag and( 139. 44± 30. 64) mg/bag in Group Auto( 2. 0 U,1. 5 U,1. 0 U) and( 184. 89±35. 58) mg/bag,( 150. 42±35. 82) mg/bag and( 120. 75±25. 24) mg/bag in Group Manu,with the difference being significant between two groups( P〈0. 01). The rates of conformance( 95%CI) to quality requirement for contents of fibrinogen were 100%( 100%—100%) in all Auto Groups and respectively 92. 50%( 87. 7%—97. 3%),95. 00%( 87%—91%),92. 50%( 84. 0%—100%) in Group Manu,with the difference being significant only between Group Auto( 2. 0 U) and Group Manu( 2. 0 U)( P〈0. 05). The work efficiency was much higher in Group Auto( 21. 25 bags/h·person) than that in Group Manu( 9. 12 bags/h·person). Conclusion Auto Cryopreper can produce cryoprecipitates with higher contents and conformance rates of factor Ⅷ and fibrinogen and has the advantages of accurate thawing temperature and cryoprecipitate volume control,less labor intensive and high working efficiency.
