凉血解毒方治疗重症肺部感染伴高热疗效观察

Therapeutic Effect of Liangxue Jiedu Prescription in Treatment of Severe Pulmonary Infection with Hyperthermia

目的:观察凉血解毒方治疗重症肺部感染伴高热患者的退热效果及其对患者肺功能、免疫功能的影响。方法:87例重症肺部感染伴高热患者随机分为对照组(43例)与观察组(44例)。对照组给予基础抗感染、呼吸支持及营养支持等对症处理,观察组在对照组治疗基础上给予凉血解毒方治疗。治疗后,比较两组患者中医证候积分、Pa O_2、Pa CO_2、氧合指数(Pa O_2/Fi O_2)、肺功能,采用临床肺部感染评分(clinical pulmonary infection score,CPIS)评定治疗前后患者肺部炎症积分的改善情况。结果:观察组有效率为95.45%,对照组为76.19%,两组有效率比较,差异有统计学意义(P<0.05)。与治疗前比较,两组患者治疗后中医证候积分、Pa CO_2及CPIS均显著降低(P<0.05);观察组以上指标降低幅度均高于对照组(P<0.05);两组患者治疗后Pa O_2、Pa O_2/Fi O_2、CD_3^+、CD_4^+均升高,观察组治疗后上述升高幅度高于对照组(P<0.05)。观察组退热时间短于对照组(P<0.05)。结论:凉血解毒方治疗重症肺部感染伴高热患者疗效显著,同时可明显改善患者免疫功能,降低中医证候积分和CPIS,提高肺功能。

Objective：To observe the antipyretic effect of Liangxue Jiedu Prescription in treatment of severe pulmonary infection with hyperthermia and its effect on the lung function and immune function.Methods：87 patients with severe pulmonary infection and hyperthermia were randomly divided into control group（43 cases）and observation group（44 cases）.The control group was given symptomatic treatments such as anti-infection,respiratory support and nutritional support.The observation group was treated with Liangxue Jiedu Prescription in addition to that of the control group.After treatment,the TCM syndrome scores,Pa O2,Pa CO2,oxygenation index（Pa O2/Fi O2）,and lung function between the two groups were compared.The improvement of clinical pulmonary infection score（CPIS）was used to evaluate the lung inflammation scores before and after treatment.Results：The effective rate of the observation group was 95.45%,and that of the control group was 76.19%.The difference of the two groups was statistically significant（P〈0.05）.Compared with that before treatment,TCM syndrome scores,Pa CO2and CPIS in the two groups were significantly lower after treatment（P〈0.05）.The decrease in the above indicators in the observation group was higher than that in the control group（P〈0.05）.After treatment Pa O2/Fi O2,CD3^＋,and CD4^＋in the two groups all increased.The above-mentioned increase was higher in the observation group than that in the control group（P〈0.05）.The antipyretic time of the observation group was significantly shorter than that of the control group（P〈0.05）.Conclusion：Liangxue Jiedu Prescription has significant curative effect on patients with severe pulmonary infection and hyperthermia,and it can significantly improve the immune function of patients,reduce TCM syndrome scores and CPIS,and improve lung function.
