腹腔镜右半结肠切除术治疗结肠癌的临床价值

Clinical value of laparoscopic right hemicolectomy in the treatment of colon cancer

目的探讨腹腔镜右半结肠切除术治疗结肠癌的临床价值。方法回顾性分析2015年2月至2017年2月76例结肠癌患者的临床资料,采取随机双盲法将其分为对照组与研究组,每组38例。对照组患者行开腹右半结肠切除术,研究组患者行腹腔镜右半结肠切除术治疗,对比两组患者手术时间、术中出血量、切口长度及淋巴结清扫数目、患者术后恢复指标(排气时间、住院时间)、并发症情况,并比较两组患者治疗满意率,随访1年,观察患者复发情况。结果研究组患者手术时间长于对照组,但其切口长度短于对照组,术中失血量少于对照组,淋巴结清扫数目多于对照组,排气时间、住院时间短于对照组,差异有统计学意义(P <0. 05);研究组并发症发生率为5. 26%,低于对照组(21. 05%),1年复发率为0. 00%,低于对照组(10. 53%),治疗满意率为97. 37%,高于对照组(78. 95%),差异有统计学意义(P <0. 05)。结论腹腔镜右半结肠切除术治疗结肠癌临床价值极高,患者手术情况良好,创伤小,恢复快,并发症发生率及1年复发率低,安全性高,且患者满意率高,值得推广应用。

Objective To investigate the clinical value of laparoscopic right hemicolectomy in the treatment of colon cancer. Methods Seventy-six patients with colon cancer admitted in our hospital from February 2015 to February 2017 were selected as the study subjects. The patients were divided into two groups： the control group（ n = 38） and the study group（ n = 38）. The patients in the control group were treated with open right hemicolectomy,and the patients in the study group（ n = 38） were treated with laparoscopic right hemicolectomy. The operative time,intraoperative bleeding volume,incision length,number of lymph nodes dissection,postoperative recovery（ exhaust time,hospitalization time,complications） were compared between the two groups. Results The operative time of the patients in the study group was longer than that in the control group,but the incision length was shorter than that in the control group,the amount of blood lost during operation was less than that in the control group,the number of lymph nodes was more than that in the control group,and the exhaust time was longer. The hospitalization time was shorter than that in the control group（ P〈0. 05）,the complication rate in the study group was 5. 26%,which was lower than that in the control group（ 21. 05%）,the recurrence rate in one year was 0. 005,which was lower than that in the control group（ 10. 53%）,the satisfaction rate of treatment was 97. 37% and which was higher than that in the control group（ P〈0. 05）,the differences were significant（ P〈0. 05）. Conclusions The clinical value of laparoscpic right hemicolectomy in the treatment of colon cancer is very high,the operation condition is good,the trauma is small,the recovery is fast,the complication rate and the year recurrence rate are low,the safety is high,and the patient satisfaction rate is high,which is worth popularizing and applying.
