中西医结合治疗WPSS相对低危组MDS临床观察

Clinical Observation on Integrated Traditional Chinese and Western Medicine in the Treatment of WPSS Relatively Low Risk Group

目的观察中西医结合治疗相对低危组骨髓增生异常综合征(MDS)的作用。方法采用随机对照的方法,共纳入病例49例,随机分为治疗组和对照组。2组均给予相同的西医标准治疗,治疗组同时给予辨证分型治疗,2组疗程均为6个月。观察2组疗效、理化检查结果、中医证候积分及造血恢复情况。结果完成研究49例,其中实验组25例,对照组24例。疗效、中医证候积分比较:治疗组有效率及中医证候积分改善均优于对照组,差异具有统计学意义(P <0. 05);且治疗组中邪实证患者联合中药治疗者获益更明显。治疗前后外周血象变化:2组患者治疗后血红蛋白、白细胞、血小板分别与本组治疗前相比均显著升高(P <0. 05);治疗组患者治疗后血红蛋白、白细胞、血小板与对照组治疗后比较均有显著差异(P <0. 05)。治疗过程中血制品输注情况:治疗组患者红细胞、血小板输注明显少于对照组(P <0. 05)。结论中医辨证治疗能有效改善相对低危组骨髓增生异常综合征的疗效、中医证候积分和造血功能,且能减少治疗过程中血制品的输注。

Objective To observe the curative effect of TCM syndrome differentiation for lower myelodysplastic syndrome. Method 49 patients were investigated with random control method,and they were randomized into treatment group and control group. The two groups were given the same basic western medicine of 6 months. The treatment group was treated with TCM syndrome differentiation and treatment. The results of physical and chemical examination,the syndromes of TCM syndrome and the recovery of hematopoiesis were observed. Results 49 cases completed the study,with 25 patients in the experimental group and 24 in the control group. The efficacy and TCM score of the treatment group was better than that of the control group,and there had significant difference. Changes of peripheral blood before and after treatment were as follows： the hemoglobin,white blood cells and platelets significantly increased in both groups before treatment. There were significant differences in hemoglobin,white blood cell,platelet between control group and treatment group after treatment. Treatment of blood transfusion was as follows： the erythrocyte and platelet transfusion in the treatment group were significantly less than that in the control group. Conclusion The treatment of TCM syndrome differentiation can effectively improve the curative effect,TCM syndrome integration and hematopoietic function of the lower risk myelodysplastic syndrome,and can reduce the infusion of blood products in the treatment process.
