加味桃红四物汤泡脚联合硫辛酸注射液治疗糖尿病足(DF)50例的临床疗效

Clinical effect of modified Taohong Siwu decoction combined with thiocic acid injection in treating 50 cases of diabetic foot(DF)

目的:观察活血化瘀法外用联合硫辛酸注射液对糖尿病足(DF)[1]的临床疗效,并评估其安全性。方法:对符合WHODF诊断标准的100例糖尿病足患者,随机分为两组:治疗组50例,空腹血糖(11.6±3.0)mmol/L,对照组50例,空腹血糖(11.2±3.0)mmol/L,对照组给与糖尿病基础治疗(包括饮食控制、胰岛素或口服药控制血糖,减压治疗);治疗组在上述基础上采用加味桃红四物汤(桃仁、红花、当归、川芎、熟地黄、伸筋草、海风藤、艾叶)水煎泡脚并联合加用硫辛酸600 mg注射液静滴,每日1次,共28天。结果在临床疗效方面治疗组比对照组具有显著差异,在足背血流动力学方面治疗组在治疗后血流速度明显好转,神经传导速度亦有明显改善,而对照组治疗前后改善不甚明显,差异有统计学意义(P <0.05)。结论:此项临床研究观察说明加味桃红四物汤泡脚联合硫辛酸注射液治疗糖尿病足方面疗效显著并能改善足背血流动力学及腓总神经及腓浅神经传导速度,具有良好的临床效果。中医治疗中活血化瘀法已成为糖尿病治疗上的常用方法。在临床观察中,中药桃红四物汤可活血化瘀,α硫辛酸静脉滴注加中药泡脚后,明显改善治疗组糖尿病足背动脉的血流速度。同时也观察到治疗组神经传导速度较对照组有明显差异,对糖尿病的神经损伤也有较好的改善作用。

Objective To observe the clinical efcacy and evaluate the safety of modifed Taohong Siwu decoction（加味桃红四物汤）combined with thiocic acid injection in treating diabetic foot. Methods： 100 diabetic foot patients who met the WHODF diagnostic criteria were randomly divided into two groups： 50 patients in the treatment group, fasting blood sugar was 11.6±3.0） mmol/L, 50 patients in the control group, and fasting blood sugar was （11.2±3.0） mmol/L, the control group was given basic treatment for diabetes （including diet control, insulin control of blood sugar, and decompression treatment）; On the basis of the above, the treatment group received modifed Taohong Siwu decoction （ 桃红四物汤 ） [（peach kernel, sa？ower, Angelica, Chuanxiong （ 川芎 ）, cooked rhubarb, stretch grass, sea breeze vine, and chinese mugwort leaf） ] combined with 600mg thiocic acid injection, once a day, for a total of 28 days. Results： The treat-ment group had signifcant diference in clinical efcacy compared with that of the control group, the blood fow rate improved obviously in the treatment group after the treatment, the nerve conduction speed also improved obviously, and the improvement of the control group was not obvious before and after the treatment. Conclusion： The clinical study showed that the combined efect of modifed Taohong Siwu decoction combined with thiocarboxylic acid injection in treating diabetic foot was remarkable and it could improve the blood fow kinetics of the foot back and the conduction speed of the total peroneal nerve and the superfcial fbular nerve, which had good clinical efect.
