原发性肝癌合并腹水临床特征研究

Study on clinical characteristics of primary liver cancer with ascites

目的:探讨原发性肝癌合并腹水的临床特征。方法:回顾性分析肝癌患者67例,根据是否合并腹水分为观察组36例,对照组31例,用Microsoft Excel、SPSS 24.0软件统计数据,比较两组的中医证型,肝功能指标ALT、AST、ALB、TBil值和生存率之间的差异。结果:观察组以脾虚湿盛型最多见,对照组以肝郁脾虚型最多见;观察组总生存率(20%)低于对照组总生存率(52%),两组比较P﹤0.05;与对照组比较,观察组AST、TBil值较高,ALB值较低(P﹤0.05);两组ALT值相比,无统计学差异(P﹥0.05)。结论:原发性肝癌合并腹水时以肝郁脾虚证为主,肝功能受到损害相对较大,病情进展快,预后差。

Objective：To study the clinical features of primary liver cancer with ascites. Methods：67 cases of patients with liver cancer were analyzed retrospectively.According to whether with ascites,they were divided into observation group（36 cases）and control group（31 cases）. The data statistics were made by Microsoft Excel,SPSS 24.0 software.The differences of TCM syndrome types,liver function indexes（ALT,AST,ALB and Tbil values）and survival rate were compared between two groups. Results：The syndrome type of spleen deficiency and intense damp was most common in observation group,while the syndrome type of liver stagnation and spleen deficiency was most common in control group.The overall survival rate of observation group was 20%,which was lower than 52% of control group;compared two groups, P〈0.05.Compared with control group,the values of AST and Tbil of observation group were higher,the value of ALB was lower（P〈0.05）. Compared ALT values of two groups,there was no statistical difference（P〉 0.05）. Conclusion：For primary liver cancer with ascites,the syndrome of liver stagnation and spleen deficiency was main;the impairment of liver function is relatively larger;the progress of disease is rapidly and the prognosis is poor.
