卡泊芬净与两性霉素B脂质体对儿童血液病患者伴真菌感染的临床疗效与安全性对照研究

Comparative Study on Efficacy and Safety of Caspofungin and Amphotericin B Liposome in Hematopathy Children with Fungal Infections

目的:研究对比卡泊芬净与两性霉素B脂质体对血液病患儿合并真菌感染的临床疗效和安全性。方法:选取2014年3月—2018年3月间收治的血液病真菌感染患儿96例资料,将其分为卡泊芬净组40例和两性霉素B脂质体56例;两组患儿治疗后的总有效率、用药期间不良反应的发生率差异。结果:卡泊芬净组患儿用药后的总有效率为65.00%略高于两性霉素B脂质体组为64.29%,但经组间比较其差异无统计学意义(P>0.05);用药间不良反应的发生率为25.00%显著低于两性霉素B脂质体组为57.14%(P<0.05);两组患儿的总有效率相当,但卡泊芬净组不良反应率明显低于两性霉素B脂质体组。结论:在血液病合并真菌感染患儿治疗方面,卡泊芬净与两性霉素B脂质体的疗效基本相似,但安全性高于两性霉素B脂质体。

Objective： To comparetive study on clinical efficacy and safety ofcaspofungin and amphotericin B lipo- some in hematopathy children with fungal infections. Methods： 96 cases of patients with fungal infections were ran- domly divided into 2 groups. Caspofungin group （40 cases） were administered with caspofungin intravenously com- pared with patients in the other group （56 cases） receiving amphotericin B liposome. Both treatments were continued un- til two weeks after fever had resolved or lesions disappeared. Adverse events and the efficacy were investigated. Re- suits： For caspofungin group （1,-40） and amphotericin B liposome group the overall effective rates were 65.00% and 64.29%, and the adverse reactions rates were 25.00% and 57.14%, respectively. There were no significant differences in the overall effective rates between two groups, but the adverse reactions rates of caspofungin group were less than those of liposomal amphotericin B group. Conclusions： In the treatment of children with hematological disease complicated with fungal infection, the efficacy of eapprofen and amphotericin B liposome were similar, but the former safety was higher than that ofamphotericin B liposome.
